Abstract [sv]

Introduktion: Stick- och skärskador är den vanligaste orsaken till skador inom tandvården och ökar risken att drabbas av blodsmitta. Sverige saknar övergripande statistik för antalet stick- och skärskador. Sedan början av 2000-talet har säkerhetsinjektionssprutor använts, vilket reducerat antalet stickskador men inte eliminerat dem. Studier pekar på otillräcklig kunskap, oerfarenhet och stress som bakomliggande faktorer till stick- och skärskador.

Syfte: Syftet var att undersöka tandhygienisters uppfattning om smittrisk vid stick- och skärskador och upplevelse om bidragande orsaker till dessa i sitt yrkesutövande.

Metod: Kvalitativ metod induktiv ansats. Fem semistrukturerade intervjuer med legitimerade tandhygienister genomfördes. Intervjuerna analyserades med kvalitativ innehållsanalys enligt Granheim & Lundman (2004) och tolkades utifrån den teoretiska krav-kontroll-stödmodellen.

Resultat: Tre huvudkategorier framkom; Förhållningssätt och kompetens, Organisation och arbetsmiljö samt Upplevelse. Tandhygienisterna hade goda grundkunskaper om smittvägar och smittförebyggande arbete och hade en god följsamhet till avvikelserapportering, provtagning och uppföljning efter stickskador. Strukturen, kompetensen och stödet från organisationen påverkade deras möjlighet att minimera risk för stickskador. Tandhygienisterna var inte beredda på den oro som uppstår efter en stickskada. Höga krav och stress var faktorer som upplevdes vara orsak till att stick- och skärskador uppstod.

Slutsats: Risken för stick- och skärskador i yrkesutövandet bland tandhygienister påverkades i hög grad av yttre fysiska och psykosociala faktorer i arbetsmiljön. Komplexiteten i yrkesrollen tillsammans med högt ställda intäktskrav skapar en ohälsosam stressande arbetsmiljö och ökar risken för tillbud.

Abstract [en]

Introduction: Injuries caused by sharp objects and needles (NSI) are the most common cause of injuries in dentistry and increase the risk of contracting bloodborne infections. Since the beginning of the 21st century, the use of safety injection syringes has reduced but has not eliminated the occurrence of NSI. Studies suggest inadequate knowledge, inexperience and stress as underlying factors to NSI.

Aim: The aim of this study was to examine the perception of the risk of infection from an NSI and contributing factors causing NSI according to dental hygienists.

Methods: A qualitative study with an inductive approach. Five semi structured interviews with dental hygienists were conducted. The interviews were analyzed with qualitative content analysis according to Granheim & Lundman (2004) and interpreted with the demand-control-support-model by Karasek & Theorell (1990).

Results: Three main categories emerged: Approach and competence, Organization and working environment, and Experience. The dental hygienists had adequate knowledge about routes of infection and infection control. They had great compliance with reporting, sampling and follow-up if an NSI occurred. The structure, competence and support from their organization affected their ability to minimize the risk of NSI. The dental hygienists were not prepared for the anxiety that occurs after an NSI. The high demands and stress were considered factors related to NSI.

Conclusion: The risk of sharp injuries among the dental hygienists was greatly affected by external physical and psychosocial factors in the workplace environment. The complexity of their profession combined with high demands of profitability creates an unhealthy stressful workplace environment and increases the risk of injuries.
