Abstract [sv]

Bakgrund: I kommunal primärvård arbetar specialistsjuksköterskan med palliativ vård där samtal med anhöriga är en stor del. Samtal med anhöriga sker både före och efter dödsfallet.

Syfte: att beskriva specialistsjuksköterskors upplevelse av samtal med anhöriga vid palliativ vård både före och efter dödsfallet i kommunal primärvård.

Metod: Studiens genomfördes som en induktiv intervjustudie. Intervjuerna analyserades genom en kvalitativ innehållsanalys.

Resultat: Kommunikation är en central del när det kommer till palliativ vård, anhöriga ska delges den information som behövs gällande den vård som ges till patienten. Genom ett bra brytpunktssamtal och tydlig kommunikation under hela processen kan missförstånd förhindras. Anhörigas delaktighet i vården är betydelsefull. Att våga lyfta samtal om döden visar sig vara en positiv aspekt. Döden är ett stigmatiserat ämne men viktigt att tala om. Arbetade år och egen livserfarenhet ger trygghet i att tala om döden och hantera samtalen med anhöriga inom palliativ vård.

Konklusion: När specialistsjuksköterskorna vågar lyfta de svåra samtalen om döden tidigt i den palliativa fasen blir anhöriga mer införstådda i situationen. Distriktsköterskorna upplever att det oftast blir fina samtal och att de flesta anhöriga är införstådda i processen. Dock finns det utmaningar. För att bemöta utmaningarna bör specialistsjuksköterskorna samarbeta och dra nytta av varandras kunskaper och erfarenheter.

Abstract [en]

Background: In municipal primary care, the specialised nurse works with palliative care where conversations with relatives are a major part. Conversations with relatives take place both before and after death.

Aim: To describe the specialist nurses' experience of conversations with relatives in palliative care both before and after death in municipal primary care.

Method: The study was conducted as an inductive interview study. The interviews were analyzed using qualitative content analysis.

Findings: Communication is a central part in palliative care. Relatives must receive the necessary information regarding the care provided to the patient. Through information about transition to end-of-life care and distinct communication throughout the process, misunderstandings can be prevented. The involvement of relatives in care is significant. To raise conversations about death proves to be a positive aspect. Death is a stigmatized topic but important to talk about. Work experience and personal life experience provide confidence in talking about death and handling conversations with relatives in palliative care.

Conclusion: When specialist nurses’ dare to raise difficult conversations about death early in the palliative phase, relatives become more understanding of the situation. District nurses find that these conversations are often meaningful and that most relatives understand the process. However, there are challenges. To address these challenges, specialist nurses should collaborate and use each other's knowledge and experience.
