Abstract [sv]

Bakgrund: Behovet av palliativ vård är stort världen över. Inom palliativ hemsjukvård har distriktsköterskan en betydande roll. För att ge en god och säker vård för patienter i livets slutskede behövs en bättre förståelse för hur distriktssköterskor själva upplever vården.

Syfte: Syftet var att belysa specialistsjuksköterskors upplevelser av palliativ vård i hemsjukvården.

Metod: En systematisk litteraturöversikt genomfördes. 19 vetenskapliga artiklar av hög kvalitet söktes upp via databaser CINAHL och MEDLINE och analyserades genom Clarke och Brauns (2006) tematiska analys.

Resultat: Specialistsjuksköterskor har en viktig roll inom palliativ hemsjukvård genom balanserandet av patienters och anhörigas behov med vårdteamets bedömningar. Det finns flera svårigheter men även möjligheter till utveckling vad gäller samverkan med andra professioner. Effektiv kommunikation är avgörande för att vården skall hålla en hög kvalitet, där en god relation med patient och anhörig är av stor vikt. Specialistsjuksköterskorna ställs inför utmaningar så som bristande kompetens, svåra samtal och beslut, allt i kombination med sin egen personliga påverkan av vården som ges.

Konklusion: Att arbeta med palliativ hemsjukvård är både givande och utmanande, dock finns det skillnader vad gäller nationellt stöd, sjukvårdssystem, ekonomi och rutiner mellan olika länder. Det behövs mer kompetens och resurser för att kunna bedriva denna vård på ett adekvat sätt.

Abstract [en]

Background: The global need of palliative care is considerable. Within palliative homecare, the district nurses have an extensive role. In order to provide good and safe care for patients at the end of life, there is a need for better knowledge about how the district nurses experience this care.

Aim: The aim was to illustrate nurse specialists' experiences of palliative care in community health care.

Method: A systematic literature review was conducted. 19 scientific articles of high-quality were sought out within the database of CINAHL and MEDLINE and were analyzed in accordance with Clarke and Braun´s (2006) thematic analysis.

Findings: Nurse specialists have an important role within palliative homecare by balancing patients´ and relatives´ needs with the care teams assessments. There are several challenges but also opportunities to improve collaboration with other professionals. Effective communication is crucial for the care to maintain a high quality, where a good relationship with the patient and relatives is of great importance. The nurse specialists are faced with challenges such as the lack of competence, difficult conversations and decisions, all in combination with their own personal influence by the care that is given.

Conclusion: To work with palliative homecare is both rewarding and demanding, although there are differences in regards to national support, health care systems and routines between countries. There is a need for more competence and resources to be able to provide this type of care in an adequate way
