Abstract [sv]

Bakgrund: Enhanced Recovery After Surgery (ERAS), ett standardiserat vårdprogram vid stor kirurgi, möjliggör snabbare återhämtning, reducerade komplikationer och minskade vårdtider. Lite är dock känt om patienternas självrapporterade hälsotillstånd när de vårdas enligt ERAS vårdprogram tillika om kön, samsjuklighet och kirurgisk metod påverkar patientens hälsoupplevelse.

Syfte: Undersöka hur patienter som genomgått kolorektal kirurgi skattar sin hälsa efter att ha vårdats i enlighet med ERAS vårdprogram. Studien syftar också till att undersöka om den skattade hälsan har ett samband med samsjuklighet, kön och/eller kirurgisk metod.

Metod: Data inhämtades genom ERAS Hälsoenkät efter utskrivning och analyserades med kvantitativ metod. Patienter som genomgick kolorektal kirurgi (n = 80) inkluderades mellan år 2016 - 2018.

Resultat: Patienterna skattade sitt hälsotillstånd, inkluderat livskvalitet och välbefinnande, som relativt gott trots besvärande postoperativ fatique, orkeslöshet och visst illamående. Oaktat kön har patienter med hög samsjuklighet samt de patienter som genomgått öppen kirurgi en signifikant sämre skattning av sitt hälsotillstånd jämfört med patienter som genomgått laparoskopisk kirurgi. Dock påverkar den kirurgiska metoden hälsotillståndet mer negativt än samsjuklighet.

Konklusion: Laparoskopisk kirurgi påverkar patientens hälsotillstånd mer positivt i flera avseenden jämfört med öppen kirurgi.Ytterligare frågor uppstår om hur vårdpersonal bättre kan förbereda och stötta patienter med hög samsjuklighet samt de som ska eller har opererats med öppen kirurgi.

Abstract [en]

Background: Enhanced Recovery After Surgery (ERAS), a standardized program for major surgery, enables faster recovery, reduced complications, and hospital stays. However, knowledge is scarce about patients' self-reported health conditions when cared according to ERAS program as well as whether gender, co-morbidity and surgical method affect their health experience.

Aim: Investigate how patients who have undergone colorectal surgery estimate their health status after receiving care in accordance with the ERAS program. Additionally, the study aims to explore whether comorbidities, gender and/or surgical method have an impact on the patient's health experience.

Method: Data were collected through the ERAS-HEALTH Questionnaire after discharge and analyzed with quantitative method. Patients undergoing colorectal surgery (n=80) were included between 2016-2018.

Results: The analysis of the ERAS-HEALTH questionnaire showed that the patients estimated their state of health relatively well, including well-being and quality of life, despite troublesome postoperative fatigue, lack of energy and some nausea. Regardless gender, patients with higher co-morbidity and those who underwent open surgery have a significantly lower experience of health compared with patients who underwent laparoscopic surgery. Moreover, it seems that the surgical method affects postoperative health to a greater extent than co-morbidity.

Conclusion: Laparoscopic surgery affects patients health positively in several respects compared with open surgery. Further questions arise of how healthcare professionals better could prepare patients with high co-morbidity as well as those patients who had undergone, or are planned for, open major surgery.
