Bakgrund: Palliativ vård syftar till att lindra lidande och främja livskvalitet i livets slutskede. I vissa situationer kan livsförlängande behandling fortsätta trots att den inte längre bidrar till symtomlindring eller förbättrad livskvalitet, utan i stället riskerar att öka patientens fysiska, psykiska och existentiella lidande. Sjuksköterskor har genom sin kontinuerliga närvaro i omvårdnaden en central roll i att uppmärksamma förändringar i patientens symtom, lidande och behov samt i att identifiera när behandlingens belastning överstiger dess lindrande effekt och vårdens inriktning behöver omprövas.

Syfte: Syftet med denna studie är att belysa sjuksköterskors erfarenheter av livsförlängande behandling inom palliativ vård.

Metod: En litteraturöversikt baserad på tolv kvalitativa vetenskapliga artiklar ur sjuksköterskors perspektiv.

Resultat: Sjuksköterskors erfarenheter av livsförlängande behandling inom palliativ vård präglas av etiska utmaningar, emotionell belastning och begränsat inflytande över beslut om fortsatt behandling. Den nära kontakten med patienten ger en helhetsbild av patientens lidande och förändrade behov samt ett ansvar att lindra symtom och bevara värdighet. Kommunikation med patienter, närstående och vårdteam är central för vårdens inriktning, samtidigt som tidsbrist, hög arbetsbelastning och bristande stöd försvårar sjuksköterskors arbete i situationer där behandling kan förlänga patientens lidande.

Konklusion: För att stärka kvaliteten i palliativ vård behövs ökad delaktighet för sjuksköterskor i beslutsprocesser samt förbättrad kommunikation och samverkan i vårdteamet. Organisatoriskt stöd är också viktigt för att möjliggöra en vård som lindrar lidande och bevarar patientens värdighet i livets slutskede.

Background: Palliative care aims to relieve suffering and promote quality of life at the end of life. In some situations, life-prolonging treatment may continue despite no longer contributing to symptom relief or improved quality of life, and may instead increase the patient’s physical, psychological and existential suffering. Through their continuous presence in care, nurses have a central role in recognizing changes in patients’ symptoms, suffering and needs, and in identifying when the burden of treatment exceeds its relieving effect and when the direction of care may need to be reconsidered.

Aim: The aim of this study is to illuminate nurses’ experiences of life-prolonging treatment in palliative care.

Method: A literature review based on twelve qualitative scientific articles from a nursing perspective.

Results: Nurses’ experiences of life-prolonging treatment in palliative care are characterized by ethical challenges, emotional burden and limited influence over decisions about continued treatment. Close patient contact provides a comprehensive understanding of suffering and changing needs, together with a responsibility to relieve symptoms and preserve dignity. Communication with patients, relatives and the healthcare team is central to the direction of care, while time constraints, high workload and lack of support complicate nurses’ work when treatment may prolong suffering.

Conclusion: To strengthen the quality of palliative care, increased nurse participation in decision-making processes is needed, together with improved communication and collaboration within the healthcare team. Organizational support is also important to enable care that relieves suffering and preserves patient dignity at the end of life.
