Abstract [sv]

Bakgrund: Prehospital akutsjukvård innebär omedelbara medicinska insatser utanför vårdenhet i komplexa och ofta oförutsägbara situationer. Sjuksköterskan ansvarar för bedömning och transport av patienter. Arbetet kräver medicinsk kompetens, teamarbete och ett personcentrerat samt etiskt förhållningssätt. Den caritativa omvårdnadsteorin betonar medmänsklighet och lindring av lidande i mötet med patienter i utsatta och akuta situationer.

Syfte: Syftet var att beskriva sjuksköterskors erfarenheter av att vårda patienter prehospitalt inom ambulanssjukvård i Norden.

Metod: Studien genomfördes som en integrerande sammanställning av kvalitativ forskning inspirerad av metasyntes. Elva artiklar inkluderades.

Resultat: Resultatet presenterades i tre huvudteman: möte med patienter i utsatta situationer, teamets betydelse och stöd från kollegor samt utmaningar och sårbarhet. Sjuksköterskor beskrev hur missbruk, hot och våld samt den fysiska arbetsmiljön påverkar vårdandet. Kollegialt stöd och teamarbete framstod som avgörande för trygghet, beslutsfattande och återhämtning. Arbetet medförde både emotionell och etisk belastning men också upplevelser av meningsfullhet och professionell styrka.

Konklusion: Sjuksköterskans arbete i prehospital vård präglas av medicinskt ansvar, etiska överväganden och möten i riskfyllda situationer som kan skapa moralisk stress. Flexibilitet och teamstöd är centrala delar för patientsäkerhet och professionell trygghet. Trots utmaningar upplevs arbetet meningsfullt genom att lindra lidande och värna om patientens värdighet.

Abstract [en]

Background: Prehospital emergency care involves immediate medical interventions outside healthcare facilities in complex and often unpredictable situations. The nurse is responsible for patient assessment and transport. The work requires medical competence, teamwork, and a person-centered and ethical approach. Caring science grounded in caritative theory emphasizes compassion and the alleviation of suffering when caring for patients in acute and vulnerable situations.

Aim: The aim was to describe nurses’ experiences of caring for patients in prehospital ambulance care in the Nordic countries.

Method: The study was conducted as an integrative review of qualitative research inspired by metasynthesis. Eleven articles were included.

Findings: The findings were presented in three main themes: encounters with patients in vulnerable situations; the importance of teamwork and collegial support; and challenges and vulnerability. Nurses described how substance abuse, threats and violence, and the physical work environment affected caregiving. Collegial support and teamwork were crucial for safety, decision-making, and recovery. The work involved emotional and ethical strain but also experiences of meaningfulness and professional growth.

Conclusion: Ambulance nurses’ work in prehospital care is characterized by medical responsibility, ethical considerations, and encounters in high-risk situations that may cause moral stress. Flexibility and team support are central to patient safety and professional security. Despite challenges, the work is experienced as meaningful through alleviating suffering and safeguarding patient dignity.
