Impact of infection control measures during the Covid-19 pandemic on psychiatric care : A qualitative study at a psychiatric clinic (English)
Abstract [sv]

Introduktion: Samsjuklighet, ökade risker på grund av psykiatrisk grundsjukdom, samt obenägenhet att söka och ta emot vård gör att psykiatriska patienter utsattes för stora risker under covid-19-pandemin. Vårdhygieniska åtgärder var problematiska att tillämpa inom psykiatrisk vård.

Syfte: Uppsatsens syfte var att erhålla information om hur psykiatriska patienter och deras behandling påverkades av vidtagna åtgärderna under covid-19-pandemin och om dessa i sin tur behövde anpassas för att fungera i psykiatrisk verksamhet.

Metod: Sju personer ur fem olika yrkeskategorier intervjuades med semistrukturerade intervjuer. Citat grupperades till utsagor som därefter lades ihop till utfall av vilka kategorier byggdes upp. Genom detta förfarande kartlades olika åsikter som fanns bland de intervjuade om den psykiatriska vården under covid-19-pandemin.

Resultat: I kategorin påverkan på patienterna rörde det sig om rädsla, överdrivet ansvarstagande, försämring i grundsjukdom, svårigheter att förstå och avsaknad av fysisk kontakt. I kategorin påverkan på behandlingen märktes svårighet att föra terapeutiska samtal, omöjlighet att använda ECT som förstahandsbehandling, ökad ångest, ej medicinskt motiverade permissioner. I kategorin påverkan på 4 smittskyddsåtgärder noterades brist på skyddsutrustning, dålig följsamhet till vårdhygieniska riktlinjer som snarare var anpassade till somatisk vård och användning av ad hoc-lösningar.

Diskussion: Det var svårt att bedriva en god psykiatrisk vård under pandemiska förhållanden på grund av vårdhygiensiska åtgärder. Inför nästa pandemi bör detta utredas närmare i syfte att anpassa åtgärderna bättre.

Abstract [en]

Introduction: Comorbidity, increased risks due to underlying psychiatric illness, and reluctancy to seek and receive care meant that psychiatric patients were exposed to great risks during the Covid-19 pandemic. Infection control measures were problematic to apply in psychiatric care.

Aim: The aim was to obtain information from health professionals about how psychiatric patients and their treatment were affected by measures taken during the Covid-19 pandemic, but also how infection control measures were affected.

Method: Seven health professionals representing five common professions in psychiatric care were interviewed using semi-structured interviews. Quotes were grouped into perceptions in turn forming outcomes from which categories were built. Through this procedure, different opinions existing among the interviewes about psychiatric care during the covid-19 pandemic were mapped.

Results: In the category impact on patients, there was fear, excessive responsibility, worsening of underlying illness, difficulty of understanding and lack of physical contact. In the category impact on treatment, difficulty in conducting therapeutic conversations, inability to use ECT as first-line treatment, increased anxiety and non- medically justified leave were noted. In the category impact on infection control measures, lack of protective equipment, poor adherence to infection control measures that were more adapted to somatic care and the use of ad hoc solutions were noted.

Conclusion: Providing good psychiatric care was problematic under pandemic conditions due to infection control regulations. Ahead of the next pandemic, this should be investigated further in order to adapt the measures in a better way.
