    Abstract [en]

    Hand hygiene compliance is one of the most important factors to prevent healtcare associated infections (HAI), which can cause unnessesary suffering, prelonged insitutional care, invalidity and higher mortality rate. It has been shown that despite regulations for hygiene compliances in healthcare, these are used unadequately. HAI affects 5-10 % of all patients in industrailized countries, in development countries the number is estimated to be up to 20 times higher. The aim of this study was to illustrate healthcareworkers hygiene compliances in patient care at a rural healthcarecentre in western India. Participant observation was used as a method, fieldnotes was taken for qualitative analysis. As a result of the analysis five fields emerged; use of handdesinfection and handwashing, complaince of gloves, the healthcareworkers clothing, use of jewlery and other hygienic aspects, which made the result. This study reveals that handhygiene complaince was poorly used by all informants, gloves was used only by one of the informants, and then only when bandaging wounds. The techniques used for both handwashing, handdesinfection and puttning on gloves was unadequate and therefor did not fullfil their purpose. Even though, the healthcare workers showed that they were aware of and thougt about hygiene in many ways.

    Abstract [en]

    Background: In encounters between nurses and patients with another language could communication problem arise. In those situations, an authorized interpreter could be needed. The need of authorized interpreters arises when the immigration increased and the society became multilingual. There could be some communication challenges during the meetings when the nurse has difficulties to understand and to pass information to a patient from a different cultural with another language background. The responsibility of the nurse and the large part of the work is to make the patient involved and agreed upon the care service that is needed. Aim: The purpose was to describe nurses’ experiences of using an interpreter in encounters with patients. Method: A literature-based study with ten qualitative articles were chosen. Result: The analysis resulted in three categories; Different opinions regarding the importance of the interpreter, unclear guidelines and that relatives should not need to act as an interpreter and eight subcategories. Conclusion: There was an obvious conclusion how important it is for nurses to make the patient involved and agreed upon in the meetings regardless from earlier background and language. The patient involvement is of great importance to nurses to secure and meet a good care. If the patient’s wishes of not using an interpreter, it could lead to a suffering for the patient.

    Bipolär sjukdom är en livslång sjukdom och livet tar en stor vändning, både för de somdrabbas samt de anhöriga. Det är av stor vikt för en sjuksköterska att kunna hantera ochstödja de anhöriga på ett bra sätt och genom att förstå de anhörigas upplevelser kan ettbättre stöd ges. Syftet med studien var att beskriva anhörigas upplevelser av att leva meden person som lider av bipolär sjukdom. Studien är litteraturbaserad och sju vetenskapligaartiklar har analyserats. Resultatet visade att de anhöriga upplevde att det dagliga livetpåverkas på olika sätt. Det visade också att behovet av kunskap om sjukdomen var stor,med hjälp av information ökade förmågan att klara av stress hos de anhöriga. De anhörigaupplevde även att det fanns ett behov av stöd framförallt ifrån professionella vårdgivare.Resultatet av studien kan bidra till en ökad förståelse hos sjuksköterskor angående deanhörigas upplevelser, känslor och behov av stöd, vilket i sin tur kan leda till att vården förden sjuke och dennes anhöriga blir bättre.

    Abstract [en]

    Background: Annually about 560 children pass away in Sweden.  Nurses have a central role in the care of palliative children. Through their care, nurses should strive for a family-centered nursing method and meet the patients and family’s values and emotions in order to create a satisfactory and meaningful end of life. The care of a palliative child stands in direct contrast to the image of a long and happy life, which contributes to great emotional challenges for nurses. Purpose: To illuminate nurses' experience of caring for children in the palliative stage. Method: A literature-based study with a qualitative approach. Results: Four themes emerged from the collected data; to create a meaningful end of life, communication and to create a connection, a struggle against lack of organizational conditions and to accept a child's passing - a problematic process. Conclusion: Nurses feel that it´s an emotionally demanding job where it´s desirable to be able to create a satisfactory and meaningful end of life. However, there is a lack of education and competence for nurses to be able to provide the desired care and to handle their emotional burden. Further education and research are required to improve the nurse's knowledge of the subject.

    Abstract [en]

    Students who stay at home from school have attracted increasing attention in recent years. Despite the Swedish compulsory school attendance in primary school and the voluntary nature of high school, more and more students give up school. Students health has an important role in promoting students school attendance, the focus is to put in effort in the early stage at absence.

    Aim: The aim of the study is to investigate the school nurses experience of working with high school students who have a long term absenteeism.

    Method: The study was conducted with a qualitative approach. The study is based on sixteen mail interviews with nurses working in high schools that were analyzed using significance analysis.

    Results: Four meanings were found describing the school nurses experience of working with high school students who have a long term absenteeism; the health dialogues enables to get to know the students, to achieve a safe relationship with students, cooperation with other professions and guardians and the importance of routines to promote students attendance.

    Conclusion: School nurses need to build a trustful relationship with students so that a safe relationship can occur. It’s important that students are being seen and heard in a respectful way, in the meeting with school nurses so that the cause of long term absenteeism appears.

    Abstract [en]

    Palliative care is a special kind of care that focuses on giving relief care to patients that are living with life threatening diseases. The disease is no longer in focus in palliative care and it is therefore offered to people who have an incurable life threatening disease. It focuses on relieving the patient’s discomfort and suffering, increase wellbeing and create conditions for an increased quality of life. Aim: The aim was to describe factors that influence on nurse’s experience of palliative care. Method: In order to give an objective and systematic conclusion of the current knowledge a literature study was chosen. Result: The palliative care is complex and nine factors emerged that play an important role in the nurse's experiences of palliative care. These factors were professional experience and skills, lack of professional experience and skills, strengthening relationships, co-worker support, and lack of ability to keep a professional distance, relative’s denial, lack of cooperation between the nurses and doctors, holistic care and symptom control and lack of holistic care and symptom control. These factors influenced nurse’s experiences, both positive and negative. Positive experiences were to find meaning in their work, satisfaction, personal development and confirmation. Meaningless, stress, inadequacy and anxiety were negative experiences.

    Abstract [en]

    Being suffered by myocardial infarction is for many people a traumatic experience and problems can also occur in the daily life, for a long time after the infarction. The aim of the study was to analyse and describe experiences when suffering a myocardial infarction, by lived experience. Autobiographies were used to collect data. Data were analysed using qualitative content analysis. All of the autobiographies were written by men. The result is represented by four main categories; Become ill, The caring relation, Needs and Coping. In the acute care the informants’ did not have control of their situation and they had a feeling of being deserted to the caring staff. The experience could also be positive, like the possibility to be able to leave the responsibility for their health to the caring staff. There was a need of sharing experiences with fellow patients and there was also a need for professional nearness and distanced nearness and they coped with the situation by looking forward, to adjust oneself and to take one step at the time.

    Fetma är ett ökande folkhälsoproblem runt om i världen. Sjuksköterskan har som uppgift att förebygga ohälsa hos patienter. Genom att veta patienternas svårigheter och möjligheter underlättar det mötet med sjuksköterskan. Men samtidigt måste sjuksköterskan vara medveten om sina attityder för att skapa en bra relation genom att samtala. Syftet är att beskriva svårigheter och möjligheter som kan påverka mötet mellan sjuksköterskor och patienter med övervikt eller fetma. Resultatet av litteraturöversikten bygger på 11 vetenskapliga artiklar. Dessa hittades både genom en systematisk sökning av databasen Cinahl och genom manuell sökning. Resultatet visar att sjuksköterskor har svårigheter att jobba med patienter med övervikt som inte har någon motivation. Sjuksköterskor bör också vara uppmärksamma på hur de uttrycker sig och bemöter patienter med övervikt. Sjuksköterskor och patienter har olika erfarenheter av varandra i mötet. Vilket kan ge en dålig start för en bra relation.

    Abstract [en]

    Background: One of the major events in human life is childbearing and the transition to becoming a parent.  It is a momentous time when women are especially vulnerable and the possibility of increased support should be available to create conditions for health and wellbeing. According to previous research, there is a clear correlation between perceived social support and numerous health benefits. Problem: However, there is barely research on pregnant women's experiences of social support during pregnancy. Aim: The aim of this study was to describe pregnant women's experiences of social support before childbirth and parenting. Method: A qualitative content analysis with an inductive approach was used as a method. Semi-structured telephone interviews were conducted and totally 15 first-time pregnant women were included in the study. Results: The results are presented in three categories with an overall theme. The result showed that social support during pregnancy had a keyrole in women's experiences of mental preparation and security for childbirth and parenthood. Conclusion: The women described the social support like a big puzzle, where each piece of the puzzle of social support contributed to a complete puzzle of mental preparation. Professional support, through for example, midwives, could facilitate social support. To meet the needs of pregnant women it is important that the professional who comes in contact with them are aware of how they can provide support, in order to facilitate social support.

    Nutritionsproblem är vanligt hos äldre på särskilt boende. Trots att problemet varit känt sedan länge, fortsätter de äldre att minska ofrivilligt i vikt. Nattfastan på särskilt boende är längre än riktlinjerna från Livsmedelsverket och Socialstyrelsen, vilket är 11 timmar.

    Syftet med studien var att belysa betydelsen av nattfastans längd i relation till de äldres viktdifferens före och efter intervention som genomfördes i två steg. Interventionen utmynnade i en ökad tillgänglighet av dryck/mat för de äldre under kväll och natt.

    Den långa nattfastan minskade efter intervention 1, de äldre blev mer viktstabila. Intervention 2 medförde ytterligare förkortning av nattfastans längd, fler äldre ökade i vikt. Intervention 2 hade effekt över tid.

    Urvalet är selektivt och litet i förhållande till kvantitativ ansats, studien ska därför ses som en förberedande pilotstudie som i liten skala prövade en arbetsrutin. Konklusion: Studien har genererat hypotesen att en förkortad nattfasta kan minimera ofrivillig viktförlust hos äldre på särskilt boende.

    Abstract [en]

    Background: Nurses use triage guidelines to sort patients who seek care in the emergency department. It is a complex situation which demands critical thinking and medical knowledge. Nurses play an important role in patients’ experience of seeking care in the emergency department.

    Aim: To gather and identify research which describes nurses’ triage work in the emergency department.

    Method: The chosen method was a literature review. The data consisted of nine qualitative and three quantitative studies.

    Results: Four themes were identified through the analysis. These where Nurses’ way of working with triage, Nurses’ work related relationships in their triage work, Nurses’ work environment in their triage work and Nurses’ attitudes to patients in their triage work.

    Conclusions: If nurses work together they can strengthen the profession and be more confident in their triage decisions. Nurses need to have good self-knowledge. If good health care is to be conducted, nurses need the administration to provide the right conditions. The study suggest that more research is needed, which can develop nurses’ role in the triage work in the emergency department.

    Abstract [en]

    According to the Swedish bureau of Statistics [SCB] there were 109 301 children born in Sweden during 2008, in addition to these there where also approximately 600 stillborn children. Intrauterine fetal death means that the child has died in the womb after the 22nd week of pregnancy. Previous research highlights the importance of the midwives´ individual approach to parents with a stillborn child. Research also illustrates how parents experienced the support given during delivery. However, there is a limited amount of research done on how midwives support the parents of a stillborn child. The aim of this study was there for to investigate how midwives experience there support during delivery of a stillborn child. The method used was qualitative content analysis with an inductive approach. Five midwives were interviewed at two maternity wards in Västra Götaland and Örebro County. The analysis process revealed three main categories; adjust to the individual, to give time and to be a tool. The result shows that midwife´s support during delivery focuses on allowing couples to control the flow of information and by being present judge the parents´ needs. It is important to give the parents time both during delivery and to spend time with the stillborn child. To be able to support, the midwife need to feel confident both personally and professionally. It was also significant what the midwife´s own view on death was.

    Bakgrund: Att få en cancerdiagnos innebär ofta en kris för den som drabbas. Kolorektalcancer är den tredje vanligast cancersjukdomen i Sverige och den tredje vanligaste dödsorsaken där män och kvinnor drabbas likvärdigt. Eftersom allt fler blir sjuka i cancer ökar också behovet av kunskap och stöd i form av vård och omvårdnad. Syfte: Att beskriva patienters upplevelser av att drabbas och leva med kolorektalcancer. Metod: Metoden som användes var en litteraturöversikt. Resultatet bygger på 10 stycken kvalitativa artiklar som sökts fram i databaserna Cinahl och PubMed. Resultat: Patienterna som drabbades av kolorektalcancer upplevde att känslorna pendlade mellan hopp och förtvivlan. Detta ledde till en känsla av förlorad kontroll över sitt liv. Information hade stor betydelse. Patienterna uttryckte ett behov av stöd från omgivningen i kampen mot sjukdomen. De upplevde också att det fanns olika strategier för hantering som var helt individuella och unika. Konklusion: Patienter som drabbas av kolorektalcancer ställs inför förändringar och starka känslor uppstår. De saknar kontroll över sina liv och är i behov av stöd. För att kunna hantera livet med kolorektalcancer behöver de använda sig av olika strategier. Sjuksköterskan behöver ha kunskap om sjukdomen kolorektalcancer för att kunna möta patienters individuella behov.

    Abstract [en]

    The waiting period for organ transplantation can be a complex situation for patients and their relatives. The purpose of the study was to enlighten the experiences of patients and their relatives when they stand in queue for organ transplantation. With a qualitative attempt four autobiographies were read and analyzed.

    The result showed that the experiences started with the information that transplantation was essential for the patients’ continuance of life, to when they were hospitalized for the operation.

    The analysis of the data resulted in five categories: The insight about the gravity of the disease, Between hope and despair, Thoughts surrounding death, Qualms of conscience and A call much longed for. The result can be used to increase the understanding for what patients and their relatives go through emotionally during their wait for organ transplantation among the nursing staff.

    Bakgrund: Endometrios är en kronisk kvinnosjukdom där livmoderslemhinnan växer utanför livmodern. Detta resulterar i inre blödningar och bäckensmärta. Syfte: Att belysa kvinnors upplevelser av att leva med Endometrios. Metod: En litteraturbaserad studie baserad på analys av 12 vetenskapliga artiklar. Resultat: Det framkom två huvudteman, Endometrios den avfärdade sjukdomen samt Faktorer som påverkar livet för kvinnor som lever med Endometrios. Resultatet från detta examensarbete påvisar att Endometrios har stor inverkan på kvinnors liv och resulterar i lidande då diagnosen fördröjs. Sjuksköterskan har en viktig roll när det gäller att ge kvinnorna rätt vård. Diskussion: Det finns ett flertal olika miljö- och stressfaktorer som inverkar på kvinnors upplevelser av Endometrios. Flertalet av dessa skulle kunna förebyggas genom utbildning av vårdpersonal och information om Endometrios till allmänheten. Publiciteten skulle i stor utsträckning minska de drabbade kvinnornas lidande. Kliniska implikationer: Endometrios klassas i nuläget inte som en folkhälsosjukdom och har inget vårdprogram. Socialstyrelsen behöver utarbeta ett specifikt vårdprogram för sjukdomen. Vårdpersonalen kan då känna minskad osäkerhet kring Endometrios.

    Abstract [en]

    Postoperative pain management (POPM) has remained an area of concern despite major efforts to improve pain assessment and management by the introduction of specified guidelines, advanced techniques for pain alleviation, and education of staff members. Different nurse specialists are involved in the perioperative care of surgical patients. It is still not known to what extent the specific information noted by the nurses about the individual surgical patient at the different steps of the perioperative management is taken into consideration so that a potentially more optimal, individualised POPM as part of acute pain services (APS) can be provided.

    The aims of the present study were to assess if information of potential value for the POPM is noted by nurse anaesthetists involved in the perioperative management of surgical patients and to what extent such information could be of value for an individualised POPM of surgical patients and if nurse involvement is of importance for the adequacy and efficacy of POPM routines in a nurse-based, anaesthesiologist-supervised acute pain service (APS) model on surgical wards.

    Semistructured interviews of nurse anaesthetists (n=40), questionnaire responses of staff members (n=375)/surgical patients (n=110) and assessment of medical records (n=135)/database data (n=222) were included for evaluation of factors of importance in the perioperative care and POPM of surgical patients. Descriptive statistics, non-parametric and parametric tests were used for the analysis of the data.

    It was found that nurse anaesthetists continuously monitor different stress evoked physiological signs induced by surgical interventions during general anaesthesia. Nurses considered the signs indicative of pain evoked stimuli and/or insufficient depth of anaesthesia. The intraoperative information of the response pattern and anaesthetic drug requirements of the individual patient noted by the nurse anaesthetist was considered at present not to be routinely taken into consideration but could be a successful strategy in an optimal multi-professional approach to postoperative pain management. The introduction of APS, using a nurse-based anaesthesiologist-supervised model, resulted in more adequate pain management routines, better patient satisfaction with information about POPM, and increased confidence in pain management among nurses on the surgical wards than was noted for the outcome data for the hospital not having introduced such an APS model. Database documentation of outcome measures of POPM for patients receiving postoperative epidural analgesia was found to provide valuable information about the adequacy of the POPM. The feedback of information from the anaesthesia services to the surgical ward nurses was found not to be efficient enough to make ward nurses properly aware of the importance of their own direct involvement in the documentation process of POPM and that such involvement could further optimise POPM outcome

    Endometrios,  en  sjukdom  med  symtom  som  smärtor  eller  blödningar  i  samband  med menstruation, börjar ofta i tonåren men diagnostiseras inte förrän flera år senare. Kvinnorna med denna diagnos beskriver ofta att de inte  fått förståelse för sina problem under tonåren. Sjukdomen kan innebära stora besvär  för den drabbade och följder på det fysiska, psykiska och sociala planet kan ses. Syftet var att studera hur unga kvinnor med endometriossymtom upplevde  skolsköterskans  stöd  under  högstadie-  och  gymnasietiden,  för  att  kunna  påverka deras livskvalitet. Via Svenska endometriosföreningens ungdomssektion i Sverige gjordes en kvantitativ   undersökning   med   kvalitativt   inslag.   Ett   frågeformulär   skickades   ut   till föreningens  59  medlemmar,  24  kvinnor  svarade.  Resultatet  visade  att  några  få  kvinnor  var nöjda med den hjälp de fått under skoltiden men de flesta kvinnorna uttryckte att de saknade kunskap,  stöd  och  förståelse  från  skolsköterskan  vilket  ibland  gav  ett  icke  tillfredsställande bemötande. Det framkom även i studien att förståelse och kunskaper saknades i vissa fall även hos  ungdomsmottagningar  och  läkare.  Tillgängligheten  av  skolsköterskan  på  de  olika skolorna  varierade.  Mer  resurser  bör  läggas  på  kunskaper  både  hos  skolsköterskor  och  de unga kvinnorna själva för att förbättra livskvaliteten hos de unga kvinnorna.

    Abstract [en]

    Background: Amongst young female adults in the western society, eating disorder is the third most common disease after asthma and obesity. About 10 % of the people who suffers from anorexia nervosa die due to the disease. Aim: This studies aim is to describe women’s experiences of being cared for Anorexia Nervosa. Method: This study was completed with a qualitative method and a narrative analysis. The gathering of data was founded on four autobiographies, written by women who has suffered from, and been cared for Anorexia Nervosa. Result: In the results, different experiences from the treatment for anorexia nervosa could be pointed out. The experience was divided into different themes and was clarified with quotations. The themes were To compete, To let go of the control, To be offended, To be seen, To get insight and To feel fear of being healthy. These themes reflected the women’s experiences from being cared for anorexia nervosa. Conclusion: This study can contribute so that health professionals receive a greater understanding about women’s experiences from being cared for Anorexia Nervosa. It could help the health professionals to promote these women’s need of care, it can also increase the women’s experience of health and well-being.

    Abstract [en]

    Background: Obesity has been rapidly increasing and so has the number of Gastric Bypass (GBP) surgeries. Not only was the individuals’ physical health affected negatively, the perceived health took its toll as well. For some individuals GBP was a last resort to a healthy life.

    Aim: The aim of this study is to illustrate what patients perceive increase health in their daily life after Gastric Bypass.

    Method: Five autobiographies were analyzed using a qualitative analysis described by Dahlborg-Lyckhage.

    Result:  The analysis showed the importance of support after GBP. It consisted of three different types: acquaintances, support groups and religion. The surgery was considered a new beginning, with a new “you” and a new healthier lifestyle. Unfortunately the food urges didn’t always disappear. To satisfy the urges some turned to alcohol and drugs. To avoid this kind of behavior patients needed realistic goals to ensure a healthy recovery.

    Discussion: To ensure the patient well-being it was important to ensure the patient has proper support. The nurse should inform the patient about support groups as well as the risks of addiction.

    Conclusion: The result illustrated what GBP patients need in order to obtain health after surgery.

    Background: Diabetes type 2 is a disease which is growing in the society and among elders. Many of these need care in the form of home healthcare. Home healthcare is a care form which is increasing as the hospitals have lesser hospital beds. This increases the demands of home healthcare and its resources. Self-care is a central part within the diabetes-care and when well-functioning it can reduce the risk of diabetes-related complications. Elderly though, have a more difficult time to maintain good self-care abilities due to the natural ageing and other possible diseases.Aim: The aim of this study was to illustrate district-nurses experiences of promoting self-care abilities among elderly patients with diabetes type 2 in home health-care settings. Method: Data were analyzed with an inductive approach by a qualitative content analysis. The data collection was made through interviews with ten district-nurses active in home health-careResults: From the analysis appeared a theme; the importance of enabling small diabetes-related lifestyle changes, and three categories; The elderlies limitations to self-care, Individualized self-care-guidance and Cooperation between different professions with seven subcategories.Conclusion: The study shows that the district-nurses have a supportive role in promoting patients self-care abilities. They cooperate with other professions to provide tools and measures needed for promoting their independency and opportunities for self-care.

    Background: Obesity is a threat against public health and has grown rapidly the last decades. Many surgeries against obesity are done in Sweden which may reduce disease outcomes as well as providing an improved quality of life. There are different types of obesity surgery where gastric bypass is the most common. Health is connected to self-perception which is important for those who choose to undergo surgery. Self-treatment are used to maintain health in daily life.

    Objective: The aim of this study was to describe people´s experiences of everyday life after a gastric bypass surgery.

    Method: The method used was a narrative analysis of six blogs written by women.

    Result: Three categories emerged from the analysis; adjust to a new daily life, difficulties to live a normal life and co-actors in the continued planning with seven subcategories.

    Conclusion: The study’s results have generated information that can be advantageously used to understand how people who have undergone a gastric bypass surgery experiencing daily life. This knowledge can help nurses to get a deeper understanding of these people, be able to apply patient-centered care and therefore operate good care.

    Encounters in nursing homes: Experiences from nurses, residents and relatives2008Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    The care of residents in nursing homes (special housing) is a major challenge for the nursing profession, especially as the population of older people has increased in recent decades. One important aim of this care is to promote the wellbeing and security of the residents. Encounters between residents, nurses and relatives are one important and frequent activity in the daily care of the residents. Available knowledge shows that the outcome of these encounters can affect the quality of care both in positive and negative ways. The meaning of these encounters is still a fairly unknown topic. The reason for conducting research on the meaning of encounters in nursing homes is mainly due to the lack of research with this focus and a need to improve understanding and knowledge about nursing home care with an emphasis on the meaning of encounters.

    The aim of this thesis was to investigate the experiences of encounters between nurses, residents, their relatives and other significant persons in order to reach a deeper understanding of the meaning of these encounters in nursing homes. A hermeneutic method was used for all four studies in this thesis (I–IV). The hermeneutic method was inspired by Hans George Gadamer and his philosophical hermeneutics. The hermeneutic method provides opportunities to interpret the meaning of human language. All data from participants in this thesis were collected through open-ended interviews with nurses, residents and relatives in four nursing homes in a municipality in Western Sweden in 2004-2005. The interviews were tape-recorded and transcribed verbatim.

    The main results of the original papers showed that the meaning of was interpreted and illuminated as the good encounter (I–IV) and the bad encounter (I–IV). In this thesis, encounters between people in nursing homes have been shown to be important for residents’ everyday life in terms of being visible and confirmed but also for nurses’ and relatives’ need to be visible and confirmed as significant persons in the care for the resident. Nurses play an important role in encounters with residents and their relatives in order to make the residents visible and to maintain his/her respect and dignity as a human being in the nursing home. It is therefore important for nurses to establish intentions for being able to contribute to the good encounter. Being present, attentive and open in the encounter with residents and their relatives and listening to their thoughts is the start of a caring action that will result in making the residents visible and confirmed.

     

    It is of great importance to realize that there are both good and bad encounters in a nursing home context, which makes certain demands on the nurses’ competence to be able to meet the resident with an open mind as an effort to achieve the good encounter, which means to see the resident as someone belonging somewhere. Accordingly, it is an urgent matter to implement knowledge about the meaning of encounters in this caring context, knowledge that can probably be transferred to nurses working in other forms of long-term care. This thesis can be seen as a contribution to generating knowledge and providing new understanding that can facilitate the development of such nursing competence; knowledge in the art of caring that gives a deeper understanding of how fundamental each encounter can be for making residents and relatives visible and confirmed as an effort to maintain good quality in the care of residents in nursing homes.

    In university undergraduate nursing programmes, didactic strategies that enable students to learn nursing skills, solve problems and develop reflective and critical thinking and practice are needed. The aim of this study was to explore how different didactic strategies support nursing students’ experiences of learning during the first year of a reconstructed nursing curriculum.

    Methods

    This study employed a qualitative approach. The data were gathered through written narratives that were analysed using qualitative content analysis.

    Results

    Nursing students’ experiences of learning through different didactic strategies, were evident in the text. These perspectives were organised into the following themes: To focus on the patient perspective and paying more attention to others, Learning from discussions and reflections on one’s own learning, Training for the professional role and becoming more courage, and Gaining insights into nursing and increasing one’s self-awareness. The education increased the students’ self-awareness, which helped them to pay greater attention to patients and their relative. During the learning process, the students became more courageous, reflected and discovered their shortcomings.

    Conclusion

    Stated didactic strategies supported a broad base of knowledge on nursing and the professional role of nurses. Educators are challenged to strengthen meaningful learning in nursing and to facilitate the progression of nursing programmes.

    Background: Overweight and obesity have increased and become a public health problem in the society. The problem can lead to suffering by the individual, both physically and mentally. In order to maintain good health and avoid overweight and obesity a healthy lifestyle has great importance. The nurse has an important role to support and motivate the patient to achieve physical and mental well-being. Aim: To illustrate women's experience of overweight and obesity's affects on their daily lives. Method: A qualitative method according to Lundman and Hällgren- Graneheim of blog texts written by women has been used. Results: In the result three categories could be identified, who described how women were affected in their daily lives. Which were "Encumbrance on the own body in the everyday life", "Impressions from the environment" and "The ever-changing self-esteem." The theme that appeared was "Physical and psychological impact in the everyday life", beacause all of the women showed different degrees of mental illness. Discussion: Women with overweight and obesity felt differently treated in the environment, their daily life was affected in such a way that their physical and mental well-being deteriorated . Conclusion: The women had a reduced physical and mental well-being, therefore, resources should be put in to increased knowledge of nurses regarding communication and mental illness.

    In spite of the fact the situation has been well known for several years, more than 25 percent of elderly patients suffer from malnutrition during hospital care. The aim of this study was to describe the nurse's assessment of nutrition status among elderly patients treated in hospital. The chosen method was a qualitative questionnaire and by using a qualitative content analysis, the answers were processed. The results were presented in seven themes; The clinical eye, Resources, Raised assessment, Registration of food intake, Mental health, Assistant nurse´s role and Lack of time. Our conclusion is that nurses have enough knowledge and aid to appraise an adequate nutrition status on their patients, but is prevented by constant lack of time resulting in nutrition questions being lower prioritized. Instead, the assistant nurse takes more responsibility for the nutrition assessment.

    Syfte: Avhandlingens övergripande syfte var att öka kunskapen om perspektiv på ungdomars hälsa som utgångspunkt för att utveckla hälsofrämjande insatser i skolan. Metod: Studie I hade en kvantitativ beskrivande tvärsnittsdesign. Datamaterialet inhämtades från 948 ungdomar (11-15 år) som analyserades med multivariat logistik regressionsanalys. Studie II hade en kvalitativ utforskande design. Datamaterialet inhämtades från 117 ungdomar (14-17 år) och analyserades med grundad teori. Studie III hade en kvalitativ beskrivande design. Datamatmaterialet inhämtades från 42 ungdomar (14-15 år) och analyserades med kvalitativ innehållsanalys. Studie IV hade en kvalitativ beskrivande design. Datamaterialet inhämtades från 27 intressenter och analyserades med kvalitativ innehållsanalys. Resultat: Ungdomarna strävar mot att lyckas och att bli framgångsrika i nuet och i framtiden. En strävan som för ungdomarna upplevs som en kamp om sin tid, en kamp som de hanterar genom att använda sig av olika strategier för att balansera skolarbete och fritid. Flickors strävan handlar om att nå framgång i skolan för att bli självständig i framtiden. För pojkarna innebär framgång att bli framgångsrika inom någon av sina fritidsaktiviteter så som sport, som visar deras fysiska styrka. Framgång i nuet är av betydelse för pojkarna till skillnad från flickorna som ser på sin framgång i ett längre perspektiv. Det hälsofrämjande arbetet bör inrikta sig på att stödja ungdomars känsla av sammanhang och stärka optimism redan tidigt i tonåren samt synliggöra organisatoriska och kontextuella strukturer och normer som påverkar flickors och pojkars hälsa och livsstil. Konklusion:Sammanfattningsvis visar avhandlingen att perspektiv på ungdomars hälsa bör förstås på olika nivåer i samhället och också inkluderar perspektiv på kön. Vidare bör faktorer som är av betydelse för ungdomars livsstil diskuteras utifrån ett salutogent perspektiv och det hälsofrämjande arbetet i skolan bör utveckla insatser med kontextuella och könsspecifika utgångspunkter för att främja flickors och pojkars hälsa och livsstil.

    Stress among adolescents in Western societies is becoming an issue of increasing concern, and the global trend of adolescents’ health shows a gradual deterioration that is independent of national differences and increases with age. The aim of this study was to explore the main concern of adolescents and about how they cope with demands in everyday life. Participants were 14–16 years old, and data were collected from three sources. A constructivist grounded theory was used as a method for generating a model of the adolescents’ description of how they cope with demands in their everyday lives. The main concern described by participants in this study was to strive to be successful and to succeed in the present and throughout their lives. We conclude that differences between girls and boys, in terms of coping with demands in their everyday lives, are important to consider in the development of health promotion initiatives targeted at adolescents.

    Background: Stress among adolescents in Western societies is becoming an issue of increasing concern and the global trend of adolescents’ health shows a gradual deterioration that is independent of national differences and increases with age. Research shows that many adolescents report high levels of stress, associated with a change in expectations about performance and an increased focus on school results. Purpose/methods: The aim of this study was to explore the main concern of adolescents to get a deeper knowledge on how they cope with demands in everyday life. Grounded theory was used as a method to generate a model. Results: The core category “striving to be successful and to succeed” explains participants’ main concern in their everyday lives as a continuous process aiming for success in the present and to succeed throughout their lives. The category is what the participants describe as a race against time and can be divided into the two conceptual categories “struggling with time” and “separating life into different worlds”, pronouncing how they struggle and cope with their main concern in order to obtain wellbeing. Our results show a difference between how girls and boys cope with their demands. Conclusions: We show that girls and boys used different strategies to cope with stress in their everyday life. The results are based on adolescent’s experiences and are therefore an important contribution for initiating interventions aimed at promoting adolescents mental health from a gender perspective.

    Background A trend of increased stress and deteriorating mental health of adolescents is a global challenge (Currier et al, 2012). Research shows that many adolescents report high levels of stress associated with an increased focus on school performance (Moknes et al, 2014). These demands generally have a stronger impact on girl’s health (Låftman & Modig, 2013) due to context and social construction of norms, values and beliefs about femininity and masculinity (Connell, 2002; Butler, 1999). The aim was to get a deeper understanding of girls and boys perceptions of how they handle demands and school-related stress. Methods This study has an explorative design and was analyzed by qualitative content analysis described by Graneheim and Lundman (2004). The participants were 42 adolescents 15 years old, interviewed in five focus groups, dived by gender from five randomly selected schools. Two additional gender mixed focus groups with 14 adolescents 15 years old, were recruited from two of the randomly selected schools. Results The results show that girls and boys handle school-related stress by using similar strategies, but in different ways. Girls express that they have to prioritize to deselect activities they use to do to handle demands from school, and boys prioritize their own activities to obtain strength to cope with demands. Girls often think about their future while boys more often live in present time, and don’t worry so much about the future. Girls receive social support and recovery from friends and family, while boys do various activities with their friends and family to get energy. Conclusions This study shows that girls and boys used different strategies to handle demand and school-related stress. The results are based on adolescent’s experiences and could therefore be an important foundation for interventions that promote adolescents capabilities to cope with increasing demands and to handle school-related stress.     Abstract [en]

    Stress among adolescents in Western societies is becoming an issue of increasing concern of adolescent’s health. The aim of this study was to gain greater knowledge about how girls and boys perceive and cope with school-related stress. Participants were 14- to 15-year-old adolescents from a medium-sized municipality in southern Sweden. The data were collected from focus group interviews. The data were subjected to qualitative content analysis. The findings show that adolescents ‘‘prioritizing the future or the present by making choices, finding their own private sphere to relax, and recovering with family and friends.’’ There were gender differences in how these strategies were used. The findings could be used for initiating and planning health promotion interventions in school with focus on supporting girls’ and boys’ equal terms to cope with school-related stress in present and for the future and to give equal condition for future studies and opportunities in life.

    A stakeholder perspective on adolescents’ needs for support to cope with school-related stressManuscript (preprint) (Other academic)
    Abstract [en]

    Background; Adolescents’ experiences of stress have increased in recent decades and is associated with an increase of psychosomatic symptoms and poorer academic achievement. Identification of ways to support adolescents’ handling of school related stress is a major challenge to promote their health and academic achievement. The purpose of this study was to describe the stakeholder’s perspectives on services that are needed for supporting adolescents in secondary school when coping with school-related stress. Methods; Representatives from the school health service and school management organization (n=23) from five secondary schools and parents (n=4) were recruited by snowball sampling. Interviews were analyzed by qualitative content analysis with an inductive approach.   Results; The study highlight different aspects of stakeholders’ perceptions of what is needed to support adolescents who experience school-related stress. Described are actions such as highlighting and changing organizational and contextual structures and developing the dialogue between the school and the home. Other described actions are to support adolescents’ abilities to plan and visualizing how time is used and to strengthen their adolescents’ belief in their own abilities. Conclusions; Our findings could be used in schools for developing interventions from a salutogenic approach to promote adolescents’ health in the present and for the future.

    Abstract [en]

    Attention Deficit / Hyperactivity Disorder (ADHD) is an attention deficit hyperactivity disorder that affects approximately 3-5 percent of school-age children. The school has an important influence on children's lives and there is a correlation between students' mental health and school performance. School nurses have an important role as health promoters, especially when children with ADHD are at increased risk of ill health and risky behavior.

     

    Aim: The aim of this study was to describe the school nurses´ experiences of working with measures for children with ADHD.

     

    Method: The study was conducted using a qualitative method. The study is based on telephone interviews with eight school nurses, which have been analyzed using qualitative content analysis.

     

    Results: Five categories emerged from the analysis, which describes the school nurses´ experiences of working with various measures for children with ADHD; Working together through dialogue, to adapt the school environment, provide guidance and advice to parents and children, to observe and implement controls and to work specifically with health promotion.

     

    Conclusion: School nurses have experience of different types of measures for children with ADHD. They stressed in particular the importance of paying attention to children with ADHD and regular health discussions.

    Background: Heart failure is a common illness, especially among elderly people. The illness is classified using the New York Heart Association Functional Classification (NYHA) and is treated using a variety of different treatment regimes. Person centered care is important to facilitate high standard care and promote the intrinsic value of the individual. Nurses and relatives play an important role in caring for people with heart failure. Aim: To describe how people over 18 years of age experience living with heart failure. Method: A review of the literature based on 15 scientific articles. Results: People experience fatigue and breathlessness as the most prominent symptoms. Breathlessness is in many cases initially difficult to attribute to heart failure and is a source of anxiety. Fatigue is experienced as a physical tiredness that leads to feelings of hopelessness. People feel misunderstood by people around them on how much fatigue affects their daily lives. Heart failure leads to physical limitations and affects daily activities negatively. This leads to feelings of limitations, poor self-image, reduced quality of live and loneliness. Conclusion: Heart failure affects people on many different levels. People experience breathlessness and fatigue to be the more difficult symptoms of heart failure and it being limiting on daily activities.

    Hospice started in the late 1960s. This cares aim is to create a good quality at the end-oflife. One of the pillars of this care is to allow the relatives to take part in the care around the patient. This study aim is to highlight the relative's needs in care for at hospice. The study is a literature review, based on scientific articles. The analysis resulted in four themes, which are information and communication, relative's involvement in care, the relative's experiences and nurse's support to the relatives. It came up that information and communication are of great importance. Both patients and their relatives wanted a little less information and many briefings. The involvement in care can give the relatives a chance to begin to grief. Relatives can need the support from the nurse in their situation. It is generally accepted that patients are well taken care of at the hospice, but what about the experience for the relative's involved in the care of their loved ones? Is it only positive to be close to the patient all the time, or are there also negative sides?

    The purpose of this study was to investigate changes in sleep quality, fatigue, mental health, and health-related quality of life (HRQoL) over a two-year period among patients undergoing peritoneal dialysis treatment at home. We further explored the extent to which sleep quality, fatigue, and mental health predicted health-related quality of life outcomes. This prospective study included 55 patients. Sleep parameters changed over two years, independently of treatment. Sleep variables at baseline, to some extent, predicted sleep quality after two years. Daytime sleepiness can be a long-term problem. Findings indicate improvements in nocturnal sleep over a two-year time period, independently of dialysis treatment. In contrast, fatigue remained unchanged over the same time period. Transplantation seems to generally benefit the outcome of HRQoL. Strategies to improve sleep and HRQoL may include systematic risk factor modification and efforts to optimise symptomatic treatment.

    Abstract [en]

    Background: Dementia is a common disease and is among the leading causes of disability amongst elderly people. Dementia disease can lead to personality changes, memory disorders, confusion and communication difficulties, amongst other things. These symptoms demand nurses to adapt their way of nursing and treating to offer the best possible care. Aim: The aim with a literature-based study is to describe nurses’ experiences with nursing and treating elderly with dementia disease. Method: In the following study a total of 10 articles of qualitative nature have been reviewed and analyzed in a structured manner. Result: The following four themes have been identified regarding the studied articles; The caring creates a variety of emotions, the importance of having prerequisites, knowledge about the individual behind the disease and the importance of being able to communicate and interact. Conclusion and clinical implications: Good treatment is a prerequisite for good care. To treat elderly with dementia the right preconditions like, time, a person-centered approach, knowledge about the disease and communication skills are necessary. The result can contribute to an increased understanding of nurses by describing other nurses' actual experiences of how nursing and treatment of elderly with dementia disease can be seen in practice.

    Abstract [en]

    Background: The human immunodeficiency virus (HIV) is a cause of chronic disease discovered in America in the 1980s. People living with HIV have since the discovery of the disease been exposed to discrimination, stigmatization and exclusion. Today effective treatment enables people with HIV to live a life without reduced lifespan. Purpose: The aim of the study is to describe people's experience of living with HIV in Sweden. Method: Narrative method based on qualitative data in form of blogs written by people living with HIV. Google database from google LLC was searched for relevant material. Result: The notification of HIV causes feelings of shock, fear and anxiety of death. Concurrent with the shocking message, a considerable need to converse occurs. However, many people also experience a fear of rejection when they tell others that they are HIV positive. This fear also have a negative impact on their sex life, since many of the them prefer to refrain from sexual relations rather than telling about the virus. Despite these difficulties, people living with HIV create strategies that make life manageable, and express a gratitude for lessons learned since exposure. Conclusion: The toughest challenge for people living with HIV is to deal with the prejudices and stigmatization by people in their surrounding.

    Abstract [en]

    Aim: To describe Swedish nursing students' perceptions of caring for dying people after the first year of a three year in a nursing programme at three university nursing schools in Sweden. Methods: Interviews (n = 17) were undertaken with nursing students at the end of their first year. A phenomenographic approach was used to design and structure the analysis of the nursing students' perceptions. Results: The analysis resulted in five categories: 1) from abstract to reality, 2) from scary to natural, 3) increased knowledge can give bad conscience, 4) time limits versus fear of end-of-life conversations, and 5) meeting with relatives. Conclusion: Nursing students need to be prepared both theoretically and within practice to encounter death and dying and to care for dying persons. By combining their theoretical knowledge of dying and death with their own encounters of death and dying people in practice, the students can be supported to develop an understanding of dying and death as a natural part of life rather than something frightening.
