    Abstract [en]

    Background: Stroke is a widespread disease and can lead to misery and disabilities. Theemotions and identity can change. Health and misery can be seen as processes and theexperiences of these are individual. Aim: Illuminate the experience of life after stroke –from the patient’s perspective. Method: Narrative method was used in order to investigatethe patient’s perspective. Seven autobiographies were read and analyzed. During theanalysis essential parts appeared which formed themes and lead to the results of this paper.Result: After the stroke weariness and other difficulties was experienced this affected thepatient’s life. Identity and body image changed. Difficulties with language appeared anddemanded a new way of communication. The stroke was shameful but in the end it offeredhope. It is meaningful for stroke patient’s to share experiences with others in similarsituations. It is difficult to manage fatigue, emotions and communication. Conclusion: Theexperience of stroke is individual and it is important for health care to support the patientin its rehabilitation and establish meeting places for sharing. Further research is necessaryto ensure good health care for stroke patients.

    Abstract [en]

    Background: Today, heart failure is a widespread disease and heart transplantation is the only long-term treatment. The nursing profession is about promoting health and well-being of the patient to strengthen the quality of life. Aim: To describe patients’ experiences in connection with a heart transplant. Method:  In the study, a qualitive method has been used with narrative material consisting of autobiographies. The study is written from a patient perspective to present patients’ experiences. Results: The result shows that patients experience, in connection with heart transplantation, a change in everyday life, uncertainty, disappointment and guilt, but then again, they also prove to be grateful to have survived the operation and their relatives’ support. Lack of support from nurses describes creating a feeling of insecurity and quality of life varied in the context of heart transplantation. Conclusion: Heart failure changes the patient’s whole life and undergoing a heart transplant is a major strain physically and mentally. The nurse must be strong in the nursing profession to support the patient to alleviate health and well-being.

    Abstract [en]

    Background: There is a shortage specialized nurses both national and international which leads to increasing waiting time for surgery and a suffering for the patient. The lack of operating theater nurses makes it important to examine how the profession is seen and if it needs to be illuminated to make it more interesting. Purpose: The purpose was to examine nurses' perceptions of the operation theatre nursing profession. Method: A descriptive cross-sectional study with a quantitative approach containing qualitative elements based on web survey with open and closed response options. Result: Nurses consider the knowledge of the operating nursing profession to be satisfactorily. The profession is considered as important, physical demanding and unknown for nurses outside the operating ward. Conclusion: The operating theater nurse is considered as important for the patient safety and is an important part of the surgical team. Despite that nurses consider themselves having knowledge of the profession the operating theater nurse is considered a hidden profession and there is a need to further make the profession visible.

    Abstract [en]

    Background: Myocardial infarction is a narrowing in one or more of the coronary arteries, which results in hypoxia in the myocardium. Many patients experience anxiety and fear when suffering from myocardial infarction. The nurse has an important function in the care of these patients. A big part of the nurse role is to support patients and care person-centered. Aim: The aim of this study is to describe nurses’ experiences of nursing patients suffering from myocardial infarction. Method: This examination paper is a literature review. The paper has a qualitative approach where eleven articles has constituted as the base to the result. Results: Three main themes are presented in the result: Positive aspects in the nursing, Challanges in the nursing and Limitations in the nursing. Nurses values to nurse person-centerered, though this is hard due to lack of knowledge, fears and shortage of time. Nurses regard bad teamwork, patients ambiguous behaviors and shortage of resources as obstructive elements in the nursing. Conclusion: Nurses that nurse patients suffering from myocardial infarction faces many challenges. Shortage of knowledge, lack of time and resources and bad collaboration in the team are a few of the obstacles that the nurses face.

    Abstract [en]

    Background: Gallbladder cancer (GC) is a relatively rare disease. To date, there are no studies describing the epidemiology of this disease in Qatar. Objective: To study the epidemiology of Gallbladder Cancer in Qatar. Methods: A retrospective analysis of the cases of GC in Hamad General Hospital in Qatar from 2009 to 2016. Results: Thirty-five patients presented with GC during the study period, 10 females (28.6%) and 25 males (71.4%). Fourteen patients (40%) were diagnosed incidentally after laparoscopic cholecystectomy, 16 (48.6%) were diagnosed pathologically, and 4 (11.4%) were diagnosed radiologically. The median age at diagnosis was 54 years (31–78). 74.3% of the disease occurred in patients less than 60 years old. Metastatic disease was discovered in 25 patients (71.4%) versus no metastasis in 10 patients (28.6%). The most common sites for metastasis were the liver (42.9%), peritoneum (25.7%), and lymph nodes (25.7%). Curative central hepatic resection was done in 8 patients (22.9%). Pathology showed adenocarcinoma in 27 patients (77.1%), neuroendocrine tumor in 3 patients (8.6%) and high-grade dysplasia in 1 patient (2.9%). No histopathology was available for 4 patients (11.4%). Twenty-eight patients (80.0%) had regular follow up, with 22 (62.9%) still alive. Six patients (17.1%) died during follow up with survival after diagnosis ranging from 42 days to 6.8 years. Conclusions: In Qatar, due to the unique demographics, GC is more common in males and younger age groups. Most of the patients present late with metastasis, but curative resection is associated with long-term survival.

    Abstract [en]

    Background: Breastfeeding brings many health benefits to the child, the mother and the community. Current recommendations in Sweden are exclusive breastfeeding the first six months. Statistics show a decreasing breastfeeding rate. The midwife's profession includes offering support and advice on breastfeeding.

    Aim: To investigate womens experience of breastfeeding, information about breastfeeding and problems related to breastfeeding.

    Method: A quantitative cross-sectional study with a deductive approach was used. Data was collected through a SIFO survey conducted by Philips Nordic. Analysis was performed in SPSS and presented by descriptive statistics.

    Results: The results of the study showed that many women were turning to health care for information and advice on breastfeeding, and that they experienced deficiencies in the information they received. Many of the women reported that problems with breastfeeding had occurred, and almost one-fifth had felt uncertain to breastfeed.

    Conclusion: Many women stopped breastfeeding earlier than recommended and experienced problems and uncertainties about breastfeeding. To get information and support, many women turned to the health care services. However, it appeared that the information and support was inadequate, which may adversely affect breastfeeding rates. Healthcare needs to improve the information and support offered to reverse the declining breastfeeding rate.

Contact with authorities, healthcare or social relations is in a shift to become increasingly digitalized, but is the entire society ready for such development?

    Abstract [en]

    Objective: During the last decade, experimental and observational studies have shown that patients with inflammatory bowel disease (IBD) and irritable bowel syndrome (IBS) may have an altered intestinal microbial composition compared with healthy individuals. However, no uniform microbial signature has as yet been detected for either IBD or IBS. This review summarizes the current knowledge of microbial dysbiosis and its potential relationship to the pathophysiology in IBD and IBS. Methods: A selective review was conducted to summarize the current knowledge of gut microbiota in the pathophysiology of IBD and IBS. Results: Experimental and observational studies provide good evidence for intestinal microbial dysbiosis in subgroups of IBD and IBS. Still, no uniform disease pattern has been detected. This is most likely due to the heterogeneous nature of IBD and IBS, in combination with the effects of intrinsic and extrinsic factors. Such intrinsic factors include genetics, the gastrointestinal environment, and the host immune system, whereas extrinsic factors include early life diet, breastfeeding, and method of infant delivery. Conclusions: Recent and ongoing work to define microbial dysbiosis in IBD and IBS shows promise, but future well-designed studies with well-characterized study individuals are needed. It is likely that the microbial dysbiosis in IBD and IBS is dependent on the natural disease course of IBD and symptom pattern in IBS. Therefore, assessment of the entire microbiota along the gastrointestinal tract, in relationship to confounding factors, symptom fluctuations, and other pathophysiological factors, is needed for further understanding of the etiology of these common diseases.

    Aim: The aim was to investigate student nurses' experiences of the clinical learning environment in relation to how the supervision was organized. Background: The clinical environment plays an essential part in student nurses' learning. Even though different models for supervision have been previously set forth, it has been stressed that there is a need both of further empirical studies on the role of preceptorship in undergraduate nursing education and of studies comparing different models. Method: A cross-sectional study with comparative design was carried out with a mixed method approach. Data were collected from student nurses in the final term of the nursing programme at three universities in Sweden by means of a questionnaire. Results: In general the students had positive experiences of the clinical learning environment with respect to pedagogical atmosphere, leadership style of the ward manager, premises of nursing, supervisory relationship, and role of the nurse preceptor and nurse teacher. However, there were significant differences in their ratings of the supervisory relationship (p < 0.001) and the pedagogical atmosphere (p 0.025) depending on how the supervision was organized. Students who had the same preceptor all the time were more satisfied with the supervisory relationship than were those who had different preceptors each day. Students' comments on the supervision confirmed the significance of the preceptor and the supervisory relationship. Conclusion: The organization of the supervision was of significance with regard to the pedagogical atmosphere and the students' relation to preceptors. Students with the same preceptor throughout were more positive concerning the supervisory relationship and the pedagogical atmosphere.

    Background: The importance of the clinical learning environment in nurse education has gained increasing attention over the last decades. However, there is a lack of research on the learning environment, its significance and meaning in specialist nurse education. Objective: The objectives of the study were to investigate specialised nursing students' experiences of supervision during clinical practice and to compare students who were satisfied with the supervision with those who were dissatisfied with respect to a)organisation of supervision and number of preceptors, as well as time allocated by preceptors for b)supervision, c)reflection, d)discussion of intended learning outcomes, and e)assessments of students' performance by preceptors. Design: This study used a cross-sectional design. Sample and settings: A convenience sample of specialised nursing students was derived from five Swedish universities in the years 2016 and 2017. Methods: Data were collected using a questionnaire. Statistical analyses and a qualitative conventional content analysis were performed. Results: While almost all specialised nursing students reported that there had been time for discussion on their performance assessment, almost half of the students reported not getting time for supervision, or time for reflections and discussions on intended learning outcomes with the preceptor. Students reporting having time allocated for supervision by preceptors were found to be more satisfied with supervision. It was described as important that the preceptor(s)acknowledged the students previous work experiences. Even though being a registered nurse, reflections and feedback were described as valuable for the students learning. Several preceptors were described as positive allowing a broader picture and different views regarding working as a specialist nurse. Conclusions: This study indicates that supervision, in terms of discussions and reflections, of specialised nursing students is significant for learning experiences and satisfaction during clinical placement. © 2019 Elsevier Ltd

    Simulation has become a widely used and established pedagogy for teaching clinical nursing skills. Nevertheless, the evidence in favour of this pedagogical approach is weak, and more knowledge is needed in support of its use. The aim of this study was (a) to explore the experiences of undergraduate nursing students when examining knowledge, skills and competences in clinical simulation laboratories with high-fidelity patient simulators and (b) to analyse these students' learning experiences during the examination. A phenomenological approach was used, and qualitative interviews were conducted among 23 second-year undergraduate nursing students-17 women and 6 men. The findings revealed that, irrespective of whether they passed or failed the examination, it was experienced as a valuable assessment of the students' knowledge and skills. Even if the students felt that the examination was challenging, they described it as a learning opportunity. In the examination, the students were able to integrate theory with practice, and earlier established knowledge was scrutinised when reflecting on the scenarios. The examination added aspects to the students' learning that prepared them for the real world of nursing in a safe environment without risking patient safety. The study findings suggest that examinations in clinical simulation laboratories can be a useful teaching strategy in nursing education. The use of high-fidelity patient simulators made the examination authentic. The reflections and feedback on the scenario were described as significant for the students' learning. Undergraduate nursing students can improve their knowledge, understanding, competence and skills when such examinations are performed in the manner used in this study.

    Abstract [en]

    Child abuse recognition and the protection of children is a global concern. In Sweden, the role of the school nurse (SN) is to promote schoolchildren’s health and development and to identify and prevent harm. The aim of this study was to describe Swedish SN experiences of suspecting, identifying, and reporting child abuse and to compare them with respect to (a) years of experience as SN, (b) age of SN, and (c) pupil population size. A descriptive design was used. Two-hundred and thirty-three SNs completed a survey detailing their experiences. Most SNs (96%) reported having suspected a child suffering from physical or psychological abuse. Approximately half of them reported occurrences of honor-related violence (54%) and of child sexual abuse (57%). SNs with less nursing experience reported significantly less recognition and reporting of child abuse. The findings indicate that experiences of child abuse are common. Thus, it is vital that SNs have the necessary competency and support to identify and report suspected child abuse.

    School nurses recognition of maltreatment of schoolchildren2019Conference paper (Other academic)
    Abstract [en]

    Background

    Human rights incorporate children and maltreatment against children is a violation of children's fundamental human rights. The Children's Convention states that children should be protected from all forms of physical and mental violence, as well as neglect and sexual abuse. Long-term effects of child maltreatment influence children’s health and academic achievement. School nurses are responsible for the identification of school age children at risk of or subjected to maltreatment, thus enforcing children’s rights.

    Aim

    The aim was to explore school nurses experiences of recognizing maltreatment against children.

    Method

    A questionnaire survey with a descriptive design was conducted. A convenience sample of 233 school nurses attending the Swedish school nursing conference in 2018 answered the questionnaire. The questionnaire contained nine questions of child maltreatment, regarding domestic violence, sexual abuse and honor related violence.

    Result

    The vast majority of school nurses, 96%, reported experiences of child maltreatment within the family. Approximately half of them, 54 %, had experiences of honor related violence to the child and 57% had experiences of child sexual abuse. Experiences of a child admitting sexual abuse in the past year were less common, 14%, but only 8 % of the nurses had made a mandatory report regarding sexual abuse during that time. Overall, the number of nurses mandatorily reporting child maltreatment is less than the number suspecting the same. 

    Conclusion

    The findings indicate that experiences of child maltreatment among school nurses are common. However, not all suspected cases, particularly in relation to child sexual abuse, were reported. More research is needed to further explore mandatory reporting and why all suspected cases are not reported.

    Abstract [en]

    Background: Pain is subjective and it means that it is an individual experience. It may feel different depending on the previous experience of the patient. No one other than the patient himself can describe the perceived pain. It is important that patients' experience of pain observed for nurses to be able to respond to their needs in the best way and thus reduce suffering. Purpose: The aim is to highlight patients' experience of postoperative pain. Method: The study is a literature review which is based on both qualitative and quantitative articles. Results: The results are reported in four different themes: To suffer from pain, Importance of information and fear of drug dependence, being worried and do not want to be a bother, to feel insecure in front of the future. Pain is an individual experience and postoperative pain may cause suffering for the patient. They do not always experience that nurses respect their expressions of pain and therefore feel forgotten because of their lack of access to information about the future. Conclusion: It is important for patients to be involved in their care, to receive pain relief and to get clear information about what affects the postoperative care. They need support from relatives but especially from all the nurses to decrease both mental and physical suffering.

    Abstract [en]

    Background: In Sweden 8000 women are afflicted by breast cancer every year. These women are subject to both physical and mental side effects and alternations. Aim: The objective of this literature review is to highlight how women in the case of breast cancer experience that their sexuality is affected by the disease and treatment. Method: The method is a review of the related literature. Results: One major theme emerges: a changed identity, and five minor themes: physiological changes, feeling unfeminine, the significance of relationship, the significance of communication in order to move on, and the significance of medical care. Conclusion: It turns out that these women experienced a loss of their sexual identity and their femininity. The deprival resulted in an inability to identify themselves with the women they once were. They also experienced physiological changes, e.g. dryness of the vaginal mucous membrane, pain from the removed breast, and less sexual arousal which impeded and reduced the sexual activity of the women. But despite reduced sexual activity, many of these women experienced acquiring a greater proximity to their partner, and that the support from a partner was regarded as important. Many of the women, though, lacked support and information from the medical institutions concerning sexual problems. They experienced that the medical staff thought that conversing upon the women’s sexuality was something embarrassing or of no importance. Since the primary objective of the nurse is to promote health and well-being, the nurse should take heed to and attend to the sexuality of the women, considering that sexuality is an important dimension in regard to experiencing health.

    BACKGROUND: In implementing new programs of care, such as person-centered care, there is a risk that the focus will be at an organizational level, instead of a level that describes what happens in the personal development among staff. The aim of this study was to describe experiences of the implementation process of a learning supporting model designed to increase patient involvement and autonomy in care. The project, which lasted 2 years, involved training sessions, supervision and reflective meetings. Over the period, the staff who participated focused on developing their dialogues with patients to make the patients aware of their own capabilities and to encourage them to be fully involved in the treatment. A reflective lifeworld approach was used. Data were collected through interviews, notes and written stories, and analyzed using hermeneutic analysis with a focus on meanings.

    RESULTS: At the beginning of the project, the participants perceived the model as abstract and difficult to understand but supervision and reflection sessions enabled understanding and changed the participants' approach to caring. The participants described the model as an approach used in challenging patients to become involved in their care and to take charge of their lives when living with a chronic life-threatening disease. The participants' experience of implementing the model has not been easy but has led to increased self-confidence and feelings of improved competence in dialogue with patients.

    CONCLUSIONS: Using the PARISH model when critically examining the results shows that in the implementation process there were some difficulties, e.g. the context was supportive and facilitating but there was no appointed facilitator. By making participation in improvement work voluntary, the impact of such work becomes less efficient, less cost-effective and probably less sustainable. Furthermore, implementation needs encouragement since changing approaches takes time and requires patience. Group supervision sessions seem an appropriate way to translate research into practice; systematic scheduled and mandatory group supervision sessions would, therefore, probably make implementation more robust and sustainable. In addition, a well-trained facilitator would be able to motivate staff to undertake daily reflection and participate in group supervision sessions. Reflection seems to be a key component in the personal learning necessary to change work routines and approaches.

    The number of persons with dementia who lives at home for a longer period of time after diagnosis is increasing. Even if the literature in the dementia field is growing, there is a need for more knowledge about everyday life of persons with a dementia disease; particularly the lived perspective of persons who live alone. The aim of this study was to elucidate the phenomenon of living alone with dementia and having a manifest care need. This phenomenological study was carried out from a reflective lifeworld approach. The data material in the study consisted of field notes from 32 visits and transcriptions from 11 tape-recorded conversations with six participants. The results reveal that the person with dementia who lives alone ends up in a vague existence where they cannot survive alone. The person’s level of activity comes to a halt and body movement becomes slower. Daily life becomes more difficult to manage and the person’s earlier natural way of relating to the world and the people around them is gradually lost. This is followed by a loneliness and forgetfulness that cloud the meaning of life. This study highlights the importance of the patient’s perspective needed to better understand the inner life of a person who suffers from dementia. This understanding is important in the organization of help and care as well as for caregivers to better understand these individuals and their needs.

    Introduction: Mental illness is a growing problem, where boys are overrepresented in the suicide statistics. Masculinity norms can have negative consequences for boys' way of expressing mental illness. Research suggests that boys only seek help if their problems are so long lasting and desperate that they don’t see any other way out.

    Purpose: To investigate how masculinity and gender norms prevent boys (age 10–25) from seeking help for mental illness.

    Method: A systematic literature review, based on 10 scientific original articles with qualitative method (focus groups and individual interviews). Datacollection took place in CINAHL, Academic Search Elite and WorldCat Discovery. The result has been analyzed with qualitative content analysis.

    Result: The result suggests that masculinity norms restrict boys from seeking help for mental illness. Factors that were decisive were stigmatization, fear of confrontation, and dealing with emotional difficulties. There was a lack of knowledge about mental illness and the threshold for seeking help was high. Boys avoid psychological problems through different external strategies.

    Conclusion: Masculinity norms have a negative impact on boys' mental illness and are an obstacle to boys not seeking help. Even structural factors emerged as an obstacle. Masculinity norms are tangible and affect boys' view of their own and others mental illness. Understanding how boys express and experience mental illness and limitations to seeking help are important to understand in order to achieve an equal healthcare. Methods regarding norm breaking strategies should be prioritized, as well as involving boys in the change and development. Research should also focus on deepening knowledge within the current context in order to be able to generalize the results.

    Background: Genital mutilation of woman is a custom practiced in several African countries, along the equator and parts of the Middle-East. Immigration from areas where this custom is common is on the rise. Midwives in Sweden are increasingly visited by women who have been victims of genital mutilation.  Guidelines and more and better education have been requested in order to approach the women in a professional way. Purpose: To highlight midwives experiences of meetings at the youth health center with women who come from areas where the practice of genital mutilation is common.  Method: A qualitative approach was chosen. Eight interviews were conducted with midwives at youth health center. Content analysis according to Hsieh and Shannon were used to analyze the material.  Results: The topic affected midwives emotionally. Midwives stressed the importance of not accepting the tradition, but felt it to be important to respect the woman and her cultural heritage. It is considered important to be gentle and responsive during the meeting and to show a good attitude towards the woman where an incorrect approach is perceived to lead to the woman feeling offended. Conclusion: Meetings with genitally mutilated women at youth health center were something that rarely occurred. Midwives knowledge of the subject was incomplete due to the lack of experience. In the current situation guidelines are not needed, but the common perception was that they will be in the future.

    Background: Breast cancer is the most common form of cancer that affects women. Around 20 women are affected each day and approximately 9000 are diagnosed with breast cancer each year. Breast cancer is an experience that involves a lot of vulnerability and deep despair. Purpose: The purpose with this study was to describe women´s experiences of living with breast cancer.Method: Literature based study with 11 qualitative articles. Results: The results showed three main categories; experiences from a new life situation, experiences of support and perspective on life. There were also eight subcategories. Discussion: The women felt that breast cancer changed their whole life. They faced several difficulties and several emotions that were difficult to handle. Many existential issues appeared and there was a need to be seen and heard.  The women wished that the support from the health care had been adapted based on their individual needs. When the patients accepted their diagnose of breast cancer, it became easier to live with it and they could return to their previous daily lives. Conclusion: Healthcare professionals have a major influence on how patients experience living with breast cancer based on the care they receive.In order to give the patients the desired care there must first be an improvement in the communication between the patient and the health care professionals.

    Introduction Caries and poor oral health is an epidemy that affects many people and especially lower classes in society with small or non exiting financial measures to seek dental care. The aim with this overview was to look at the connection between bad socioeconomic aspects and poor oral health.

    Method 10 scientific articles that treated the subject socioeconomic aspects and poor oral health were searched for in the database PubMed. Only articles regarding Swedish oral helath were chosen, all ages included.

    Result This study showed that almost every article brought up the correlation between bad socioeconomic relations and bad oral health. More knowledge about socioeconomic inequalities is needed in order to work in a preventive way.

    Discussion To even out socioeconomic inequalities knowledge is needed and a less expensive dental care because the reason low income gropus avoid seeking dental care is because they can not afford it even if they need it. Mapping out tooth issues is helpful to be able to detect trends over a long time in order to work preventively.

    Abstract [en]

    Tau accumulation affecting white matter tracts is an early neuropathological feature of late-onset Alzheimer’s disease (LOAD). There is a need to ascertain methods for the detection of early LOAD features to help with disease prevention efforts. The microstructure of these tracts and anatomical brain connectivity can be assessed by analyzing diffusion MRI (dMRI) data. Considering that family history increases the risk of developing LOAD, we explored the microstructure of white matter through dMRI in 23 cognitively normal adults who are offspring of patients with Late-Onset Alzheimer’s Disease (O-LOAD) and 22 control subjects (CS) without family history of AD. We also evaluated the relation of white matter microstructure metrics with cortical thickness, volumetry, in vivo amyloid deposition (with the help of PiB positron emission tomography -PiB-PET) and regional brain metabolism (as FDG-PET) measures. Finally we studied the association between cognitive performance and white matter microstructure metrics. O-LOAD exhibited lower fiber density and fractional anisotropy in the posterior portion of the corpus callosum and right fornix when compared to CS. Among O-LOAD, reduced fiber density was associated with lower amyloid deposition in the right hippocampus, and greater cortical thickness in the left precuneus, while higher mean diffusivity was related with greater cortical thickness of the right superior temporal gyrus. Additionally, compromised white matter microstructure was associated with poorer semantic fluency. In conclusion, white matter microstructure metrics may reveal early differences in O-LOAD by virtue of parental history of the disorder, when compared to CS without a family history of LOAD. We demonstrate that these differences are associated with lower fiber density in the posterior portion of the corpus callosum and the right fornix.

    Background: The total number of people with dementia symptoms is expected to double every 20years and there will also be an increase in the number of older immigrants in several countries. There are considerable deficiencies in the present knowledge of how to conduct well-functioning health care for immigrants with dementia symptoms. The aim of this study was to explore caring and uncaring encounters between assistant nurses and immigrants in two group homes for persons with dementia symptoms in Sweden: a Finnish-speaking as well as a Swedish-speaking context. In addition, this study aims to describe how caring and uncaring encounters are manifested in these two contexts according to Halldorsdottir's theory of Caring and Uncaring encounters. Method: Descriptive field notes from 30 separate observations were analyzed using qualitative deductive content analysis. Results: The main category caring encounters focused on reaching out to initiate connection through communication, removing masks of anonymity by acknowledging the unique person, acknowledgment of connection by being personal. Reaching a level of truthfulness by being present and showing respect, raising the level of solidarity by equality and true negotiation of care, based on the residents' needs. The main category, uncaring encounters, focused on disinterest in and insensitivity towards the other, coldness in the connection and lack of humanity in care situations. The observations showed that caring encounters occurred more in the Finnish-speaking context and uncaring encounters more often in the Swedish context. Conclusion: Encounters could be caring, uncaring, and carried out using a person-centered approach. Communication and relationships could be facilitated using the same language but also through learning to interpret residents' needs and desires.

    Abstract [en]

    Swedish school has been subject to a lot of criticism during many years. Much has been about poor academic achievement, grade inflation and the lack of equivalence in ratings. When the reform in 2011 was launched, with the aim of overcoming these particular problems, clearly formulated knowledge requirements was to increase the equivalence and a new grading scale, with more steps would motivate diligent study and combined with the clearified knowledge requirements would also curb grade inflation. The purpose of this study is to investigate if it has become more difficult to reach the highest grade level and if grade inflation may be stopped, but also if there were other implications for teaching. Was it, for example, also harder to reach the limit for approval?

    To seek answers to these questions I have choosen  to interview some colleagues about their perception of the impact of the reform on grades and teaching. The interviews were documented by audio recording equipment and then transcribed into text documents. In the interpretation and analysis of the statements has three methodological tools been used. Meaningconcentration has been used to retain the essence of statements without revealing details about the teacher's subject area. A hermeneutic perspective has been practiced to interpret what was said and actiontheory based on von Wright's four determinants have been used to further access the motives behind the statements.

    Six teachers were interviewed and almost all believe that it has become more difficult to reach the highest grade level in their courses after the reform in 2011. Also, it appears that the new grading scale is well recieved, while the fact that all the grading criteria must be met in order to achieve a certain grade level, is met with some aversion. Several teachers also feel themselves controlled by the new syllabuses and find it difficult to keep up with the course within the allotted class time. I launch my own hypothesis that they might miss certain things they see as essential for the subject because they were omitted when they were not considered possible to evaluate. Training seminars is the only short term solution I can see to get closer to an equivalent rating. I would also like to se a measurement of when equivalence can actually be considered as reached.

    Distriktssköterskan har en viktig roll i mötet med patienter som lider av hälsoproblem. Förskrivning av fysisk aktivitet (FaR) är en del i hälsosamtalet med patienten. Syftet med studien är att beskriva distriktssköterskans upplevelser av att använda fysisk aktivitet som ett redskap i hälsoarbetet. Datamaterialet består av intervjuer med åtta distriktssköterskor. Intervjuerna bandinspelades och transkriberades ordagrant. Intervjuerna har analyserats enligt kvalitativ metod med en fenomenografisk ansats. Efter genomförd analys kunde fem beskrivningskategorier urskiljas: följsamhet, omvårdnadsåtgärder, motivation, främjande faktorer samt svårigheter. Dessa kunde relateras med tio subkategorier. Resultatet visar ett flertal omständigheter som påverkar hur distriktssköterskorna upplever fysisk aktivitet på recept som ett redskap i hälsoarbetet. Distriktssköterskorna upplever att följsamheten hos patienten har betydelse för deras egen motivation till förskrivning. Den ekonomiska aspekten upplevs ur flera olika synvinklar som har betydelse för arbetet. De upplever att samarbetet mellan olika yrkesprofessioner samt andra instanser är betydelsefullt. Distriktssköterskorna i studien menar att det är få patienter som visar det resultat som förväntas efter förskrivning av FaR. Trots det upplever de att arbetet kring FaR är viktigt och motiverande.

    Abstract [en]

    Abstract. Discrepancies in perceived parenting and parental roles across European countries could be due to the use of different assessment techniques or due to mean level differences in the authoritative, authoritarian, or permissive parenting styles. The present study aimed to examine the psychometric characteristics of the Parenting Styles and Dimensions Questionnaire (PSDQ; Robinson, Mandleco, Olsen, & Hart, 1995, 2001) in a sample of 225 Greek, 301 Italian, and 279 Swedish adolescents aged 16?19 years, who evaluated their father?s and mother?s parenting styles during their childhood. A confirmatory factor analysis (CFA), multigroup CFA, and modified version of the correlated uniqueness model were used to evaluate the structure and invariance of the scale across countries. Measurement and structural invariance was found in the 8-item authoritative scale and 6-item authoritarian scale. A mixed ANOVA (Country ? Style ? Role) showed that Swedish mothers scored lower than Italian or Greek mothers, and that, in the three countries, mothers were perceived as more authoritative than were fathers.

    The Swedish Tax Agency is one of the most important governmental agencies as it affects all individuals of the society in some way. This studies purpose is to create a deeper understanding of why there is a discrepancy in the answers from the Tax Agency employee survey in 2013, regarding the experience of job satisfaction in relation to the perception of the Swedish Tax Agency as an attractive workplace and to the view of recommending the workplace to other people. The study uses a qualitative approach and is based on the concept of job satisfaction, which is regarded as the attitudes people feel towards their work. By means of 12 semi-structured interviews with caseworkers at a tax office in Western Sweden, an attempt was made ​​to explain the discrepancy through analysis of employee perceptions. The empirical material was analyzed by phenomenological data analysis and the conclusions are that there is a certain degree of dissatisfaction among case workers relating to factors such as wages, lack of career paths and the experience of other people's perception of one's work and workplace.

    BACKGROUND & AIMS: We have limited knowledge about the association between the composition of the intestinal microbiota and clinical features of irritable bowel syndrome (IBS). We collected information on the fecal and mucosa-associated microbiota of patients with IBS and evaluated whether these were associated with symptoms.

    METHODS: We collected fecal and mucosal samples from adult patients who met the Rome III criteria for IBS at secondary or tertiary care outpatient clinics in Sweden, as well as from healthy subjects. The exploratory set comprised 149 subjects (110 with IBS and 39 healthy subjects); 232 fecal samples and 59 mucosal biopsy samples were collected and analyzed by 16S ribosomal RNA targeted pyrosequencing. The validation set comprised 46 subjects (29 with IBS and 17 healthy subjects); 46 fecal samples, but no mucosal samples, were collected and analyzed. For each subject, we measured exhaled H2 and CH4, oro-anal transit time, and the severity of psychological and gastrointestinal symptoms. Fecal methanogens were measured by quantitative polymerase chain reaction. Numeric ecology analyses and a machine learning procedure were used to analyze the data.

    RESULTS: Fecal microbiota showed covariation with mucosal adherent microbiota. By using classic approaches, we found no differences in fecal microbiota abundance or composition between patients with vs without IBS. A computational statistical technique-like machine learning procedure allowed us to reduce the 16S ribosomal RNA data complexity into a microbial signature for severe IBS, consisting of 90 bacterial operational taxonomic units. We confirmed the robustness of the intestinal microbial signature for severe IBS in the validation set. The signature was able to discriminate between patients with severe symptoms, patients with mild/moderate symptoms, and healthy subjects. By using this intestinal microbiota signature, we found IBS symptom severity to be associated negatively with microbial richness, exhaled CH4, presence of methanogens, and enterotypes enriched with Clostridiales or Prevotella species. This microbiota signature could not be explained by differences in diet or use of medications.

    CONCLUSIONS: In analyzing fecal and mucosal microbiota from patients with IBS and healthy individuals, we identified an intestinal microbiota profile that is associated with the severity of IBS symptoms.

    Background: Malignant hematologic diseases can occur both among children and the elderly. Successful communication is a central part of satisfying and safe person-centered care, hence nurses need to be informed about how patients experience communication with nurses. Aim and objectives: To examine patients’ experience of communication with nurses during their hospital stay while being treated for malignant hematologic diseases. Method: Quantitative design with qualitative elements - a prospective cross-sectional study where data comprised a patient questionnaire survey of their treatment in a hematology ward at a hospital in Western Sweden. Results: Patients were overall satisfied with the communication with nurses; however, signs of insufficient nursing staff and stress were eminent. Patients desired more time for conversations with nurses. Occasionally other patients were present in shared patient rooms while private matters were discussed with nurses and doctors. Discussion: Integrity emerges as an important aspect, as conversations with nurses in shared patient rooms can leave patients exposed. Conclusion: Larger comparable reoccurring studies are needed to outline patients’ experiences of communication with nurses in different wards, to get a clear perception of what can be improved and how. This research would provide consistency and improvements related to communication between patients and nurses.

    The study of Gender does not often explore masculinity, especially with regards to their role in reproduction in society. The social psychology perspective of the daily socialisation that occurs between media and the social structure is therefore the outset of this study. A cultural representation of men has been described from a dataset of 80 analysed films, and 239 analysed male characters from Sweden and America during the 1950s and 2010s. A qualitative- quantitative content analysis was conducted to place the analysed male characters in already existing categories of masculinity. The results showed that masculinity does not have enough space in today’s society. Due to these findings, many men no longer fit the profile into the existing categories of masculinity. The biggest differences between men from the 1950s and 2010s, was a shift in the importance of family life. Over the years, men have developed a more emotional and sensitive side to themselves. When comparing the difference between the American and Swedish man; The American man tended to dominate his work life while being a family father. Whereas, the Swedish man lived a life of a bachelor and devoted a great deal of time to his friends.

    I artikeln redogörs för de viktigaste faktorerna som påverkar brukarnöjdhet inom svensk äldreomsorg på basis av statistiska analyser av primärdata i Socialstyrelsens och SKL:s årliga undersökning Öppna jämförelser samt fältstudier. Resultaten visar på den stora betydelsen av goda relationer mellan medarbetare och brukare präglade av respekt, trygghet och inflytande.  I artikeln redogörs också för de fem viktigaste komponenterna i det som kallas brukarorientering nämligen uppgiftsfokus, individfokus, känsla, samarbete och tidsanvändning. Syftet med denna artikel är att uppmärksamma beslutsfattare och äldreomsorgsadministratörer på betydelsen av att sträva efter en hög relationskvalitet och att undvika ett överdrivet fokus på att nå goda rankningresultat, särskilt som dessa rankningar har visats överdriva skillnader i kommuners äldreomsorgskvalitet.

    When the patient wishes to die at their own home requires that the advanced care also moved to their home. District nurses have primary care responsibilities in palliative home care and may experience ethical, moral and communicative difficulties. For a good palliative care requires that district nurses is involved, have the experience and have advanced skills in palliative care. The purpose of this study is to illuminate district nurses' experiences of caring for patients in the final stage in palliative home care. Nine district nurses were interviewed. To analyze the data, a qualitative content analysis with an inductive approach was applied. The analysis resulted in four main themes with its eight sub- themes. Main themes titled; Being able to collaborate and cooperate in care, Being able to care for relatives, Being able to manage negative emotions and Able to create a good care environment. The conclusion was that, a good teamwork is important for district nurses but having different care providers can complicate caring. It is important for district nurses to meet relatives and receive grief when grief could come as dissatisfaction. This could affect the nursing staff negatively. It is important that district nurses are supported in the sense of inadequacy and emotional reactions. Further research may be aimed at relatives ' point of view in order to understand family members' reactions.

    Abstract [en]

    Obstructive sleep apnoea (OSA) is a prevalent breathing disorder that decreases the quality of life, and may lead to severe comorbidities. The complex genetic components of OSA pathogenesis advocates for genetic association studies to understand the underlying mechanisms. The tumor necrosis factor  (TNF-α -308G/A promoter polymorphism has been associated with OSA susceptibility in several populations, however this relationship has not been studied in the Swedish population. The aim of this study was to assess the genotype- and allele frequencies of TNF-α -308G/A polymorphism in a Swedish OSA cohort, and look for potential associations with clinical parameters related to OSA.

    Genomic DNA samples (n=326) from the Swedish RICCADSA cohort was genotyped with PCR-RFLP. PCR fragments were digested with the restriction enzyme NcoI and analysed in an automated Fragment Analyzer. The results indicated no association between the TNF-α -308G/A polymorphism and OSA susceptibility in this cohort, and no association between genotypes or allele carriage regarding severity, BMI distribution, circulatory TNF-α or comorbidities was found.  

    The results were potentially obscured by the subjects also having coronary artery disease (CAD), which may involve similar mechanisms as OSA. The cohort did not include samples from a matched healthy Swedish control population, meaning that the results may not reflect the actual relationship between TNF-α -308G/A polymorphism and OSA. In future work, inclusion of genotypes and clinical data from a matched healthy control group are required to investigate the potential relationship between the TNF-α -308G/A polymorphism and OSA in the Swedish population.

    INTRODUCTION: Among the world's regions, the WHO European Region has the lowest rates of exclusive breastfeeding at age 6 months with around 25%. Low rates and early cessation of breastfeeding have important adverse health consequences for women, infants and young children. Protecting, promoting and supporting breastfeeding are a public health priority.

    OBJECTIVES: National breastfeeding data and monitoring systems among selected European countries and the WHO European Region are compared. Mechanisms for the support, protection and promotion of breastfeeding are reviewed and successes and challenges in implementation of national programs are presented.

    METHODS: National representatives of national breastfeeding committees and initiatives in eleven European countries, including Belgium, Croatia, Denmark, Germany, Ireland, Italy, The Netherlands, Norway, Spain, Sweden and Switzerland, participated in a standardized survey. Results are evaluated and compared in a narrative review.

    RESULTS: Variation exists in Europe on breastfeeding rates, methodology for data collection and mechanisms for support, protection and promotion of breastfeeding. Directly after birth, between 56 and 98 % of infants in all countries were reported to receive any human milk, and at 6 months 38-71% and 13-39 % of infants to be breastfed or exclusively breastfed, respectively. National plans addressing breastfeeding promotion, protection and support exist in 6 of the 11 countries.

    CONCLUSIONS: National governments should commit to evidence-based breastfeeding monitoring and promotion activities, including financial and political support, to improve breastfeeding rates in the Europe. Renewed efforts for collaboration between countries in Europe, including a sustainable platform for information exchange, are needed.

    Background: Cardiovascular disease is one of the biggest global health problems and affects both the patient's perception of life and quality of life. There are several measures the patient can take to influence and improve his or her life situation, both to prevent and when illness has occurred. Lifestyle plays a crucial role in health, methods for motivating lifestyle changes are therefore important for the nursing. Motivational interviewing (MI) can help patients understand the effect on lifestyle and with this make lifestyle changes that improves health. Aim: Examine the effect of motivational interviewing in nursing with a focus on lifestyle change and quality of life associated with cardiovascular disease. Method: A systematic review, including ten randomised controlled trials. Findings: From the analyzed data, the following six main themes emerged: Increased physical activity, weight loss, reduced smoking, increased self-care, experience quality of life and the time of treatment. These subjects showed that MI can be an effective way to succeed in lifestyle changes and increase self-care and quality of life. Conclusion: From the analyzed data material, the following six main themes emerged: Increased physical activity, weight loss, reduced smoking, increased self-care, perceived quality of life and the importance of treatment time. These themes showed that MI can be an effective method for successfully implementing lifestyle changes and increasing self-care responsibility and quality of life.

    Introduction: Infection control in animal health care is, as in human health care, most important to prevent and fight antibiotic resistance. Animal health can also influence public health. Basic hygiene routines; hand hygiene, routines for proper attire, and routines for personal protective equipment (PPE), such as protective aprons and gloves, represent the simplest and most important infection control and prevention measures. High compliance to these routines is important to achieve. However, due to few scientific publications in the animal health care arena, customized and efficient efforts have not been fully identified for this environment. Therefore, optimal oversight and compliance assessments are difficult to achieve. Similar to human health care, several infection control and prevention measures and responsibilities are addressed by nurses.

    Objectives: To study compliance to basic hygiene routines and assess whether compliance differs between different animal health care professions, such as nurses versus veterinarians.

    Method: A quantitative web-based survey about basic hygiene routines, was used to investigate compliance to basic hygiene routines among animal healthcare providers. The eligible respondent sample was comprised of 360-2600 providers. Difference in compliance among professions within animal health care was estimated using SPSS statistical analysis software.

    Results: Of a possible 360-2600 possible respondents, 313 animal healthcare providers responded to our web-based survey. Significant differences in compliance among professions were not found (between veterinarians and veterinary nurses with a degree, and veterinarians versus nurses without a degree). Higher compliance was observed at larger animal hospitals and among personnel educated at their work place in basic hygiene routines. Hand disinfectant use before contact with patients was most often missed, as well as not wearing a protective apron or coat if risk for contact with body fluids was evident.

    Conclusion: In contrast to previous practical experience and reports, our results showed no significant difference in compliance between animal healthcare veterinarians and nurses. Given our null findings, more studies are needed to identify hidden factors influencing compliance among animal health care workers, as well as attention to representative sampling. Even so, based on our results, potential interventions include enhanced work-based education programs on infection control and prevention, with attention to smaller (small to medium sized) animal health care clinics. Focus for such interventions should also address hand disinfection before contact with patients. In addition, more effective monitoring is required.

    Introduction: The society develops and sets high standards on individuals and that make the stress increase. Long-term stress can lead to public health diseases such exhaustion syndrome and depression. To provide stress’s negative effect on health there is a need of reducing programs and interventions such meditation/mindfulness that can be included in the everyday life.

    Aim: The aim of this literature study is to investigate what scientific evidence exists about how meditation / mindfulness can help individuals manage and reduce stress, and then increase well-being and health.

    Methods: The method is a literature study, which means that the articles that are included in the study have been critically reviewed in a literature review. Ten articles are included in this study and they are divided into four themes that are reported in the result.

    Results: Meditation / mindfulness can be effective methods of managing and reducing stress, but all studies were not assured. The result is divided into four different themes, positive with meditation / mindfulness, doubts about meditation / mindfulness efficiency, mindfulness-based stress reduction (MBSR) and online programs. Some studies suggested that it could increase well-being and reduce stress. While others were uncertain of its benefits. Mindfulness-based stress reduction (MBSR) showed clearer results about stress reduction programs and online programs. MBSR can be used by both sick and healthy people. Online programs are effective both cost-effectively and that the individual can reach the programs from home.

    Discussion: There was not as clear evidence of meditation/mindfulness effectiveness in stress management as expected. All studies did not agree that it had any effect but in spite of doubt, more research is needed before ruling out as a good method. How effective it is for an individual may vary because the programs require training, understanding and trust. Meditation/mindfulness doesn’t help every individuals to manage their stress, but for some it does and for some it may be a good method while waiting for professional help.

    Abstract [en]

    We studied the experience of professional support among first-time mothers in relation to a scale measuring professional support in maternity care. We used a qualitative study with both an inductive and deductive approach and interviewed nine mothers. Our findings, both inductive and deductive, suggest that first-time mothers expect professional support in their transition into motherhood, building a bridge between two worlds. The first meeting, acknowledging individual needs, and supporting partner participation were important for good support. Maternity care should be organized with a focus on availability and professional support for mothers and the increased participation of their partners. Our scale of measurement can be useful but needs some development.

    Aims and objectives: To describe the experiences of school nurses working with overweight schoolchildren. 

    Background: School nurses play an important role in health promotion of overweight children. Lifestyle changes and interventions to address being overweight can improve health outcomes and decrease the risk for future health problems. 

    Design: A descriptive and qualitative design with a phenomenological approach was used. Data were gathered through interviews with school nurses working with overweight schoolchildren in Swedish elementary school; the data were subsequently analysed for meanings. 

    Results:Working with overweight children was perceived as demanding and challenging by the school nurses who found conversations on this topic emotionally loaded and complex. In addition, the school nurses needed to be sensitive and supportive to succeed in their support for a healthier everyday life for the schoolchildren. It was stated as important to find ways to break the child ’s weight gain and to cooperate with the parents in this work. The children ’s decrease in weight was experienced to be more successful when making small, step-by-step changes together with the child and his or her parents.

    Conclusions: This study concludes that health talks about being overweight may be a challenge for school nurses. Strategies used to manage and succeed in this work included engaging in motivational conversations, working step by step and cooperating with the child’s parents. Furthermore, the nurses experienced that they needed to provide emotional support for overweight children during school time. 

    Relevance to clinical practice: The school nurses’ health promotion needs to focus on how to break weight gain in overweight children. In this work, the nurses’ sensitiveness seems pivotal. Further research is needed on school nurses’ work with health promotion and support of overweight children concerning how to perform efficient communication and cooperation with the children and their parents.

    Abstract [en]

    Background: The physical environment has an impact on wellbeing. School nurses can use this to promote students’ health. Aim: The aim of this study is to elucidate school nurses’ perceptions of using the physical environment at their office, as a tool in nursing. Method: A qualitative interview study was conducted with five Swedish school nurses, using semi-structured questions. Data were analysed using qualitative content analysis. Results: Three categories (offering a health environment for wellbeing, being present in the environment for the children and using the environment for health education) and six subcategories emerged during analysis. Conclusions: School nurses made their office a place for wellbeing by using it for health information, as a sanctuary and as an environment that supports children’s need for integrity. How the school nurses used their environment depended on their personality. Obstacles included people in the school who had little knowledge of what school nurses’ work consists of and also the difficulty of appearing available to the children.

    Background: Pregnancy can be physically and emotionally stressful for the parents, which means they need adequate professional support. Easy-to-use, validated scales are important in order to develop professional support in Antenatal care as well as in research.

    The aim: Our aim was to illuminate pregnant women’s experience of professional support at the Antenatal care, in relation to the Mother-Perceived-Professionals-Support (MoPPS) scale.

    Method: A qualitative study design using the method “Think aloud” with both inductive and deductive approaches, was used. Five first-time mothers were interviewed with open questions followed by questions related to the MoPPS scale items. Data was analyzed using inductive and deductive qualitative content analyses.

    Results: The inductive analysis resulted in one theme: “Professional support from midwives made women created a feeling of security and unique or rejected and lonely during pregnancy” and three categories: “Continuity and competence”, “Perceiving trust or not” and “Parental groups or individual visits”. The deductive analysis described the mothers’ understanding of each item. However, coherence between the inductive and deductive analyses varied and the MoPPS-scale needs development.

    Conclusion: The result shows that women's experience of professional support affects their sense of feeling safe or lonely during pregnancy. Important for midwives were to meet the women’s unique individual needs. The MoPPS scale was considered to be relevant and easy to understand, but it needs development to include questions about continuity, parental groups and the perception of midwives’ competence, which all were important for the women during their pregnancies.

    Clinical implications: To offer adequate professional support for women during pregnancy, midwives need to meet the women’s unique individual needs with both medical and supportive knowledge.

This tends to have a negative effect on their future adult life, both economically as well as socially. In Sweden their is a well established, national, municipally arranged, adult education which gives a second chance to study for some of these young adults. The purpose of this collection of case studies (Flyvbjer, 2006) is to analyse and shed some light on the social and educational factors that might benefit these adult students to achieve their goals and to empower themselves (Lightfoot, 1985). It is by using Deweys holistic approach to human language and experience and combining external and internal life, that the interviews was collected from three students who attend some additional, educational support. The analysis of the material was executed in five steps and following the method of content analysis (Kvale, 1997). In my results chapter I present excerpts and analysis grouped under five thematic headlines;1. School as the setting for social identity making. 2. Socio-economical factors and flexibility3. School as a positive community. 4. Relations building on feedback and mutual respect.5. Focusing on education and empowerment.

    Bakgrund: Övervikt och fetma är ett globalt, betydande växande hälsoproblem som verkar vara ett särskilt problem i familjer som lever under låga socioekonomiska förhållanden och ofta finns det ett samband mellan moderns och barnets övervikt och fetma.

    Syfte: Att undersöka vilka faktorer som kan leda till övervikt och fetma hos mödrar och barn med låg socioekonomisk status.

    Metod: Denna studie är en analytisk litteraturstudie med ett systematiskt tillvägagångssätt. Elva vetenskapliga artiklar har granskats och analyserats i syfte att finna möjliga samband mellan mammors och barns fetma.

    Resultat: Det finns flera multifaktoriella orsaker som kan bidra till ohälsa hos föräldrar, framför allt ensamstående mödrar. Inkomst, utbildning, familjeförhållanden, kunskap och känslomässiga aspekter är faktorer som kan ligga bakom övervikt och fetma hos både mamma och barn.

    Slutsats: Slutsatsen av denna litteraturstudie var att övergripande förstå och identifiera faktorer som kan kopplas till övervikt och fetma i familjer med låg socioekonomisk status och därigenom få kunskap om var insatser kan implementeras
