    Abstract [sv]

    Bakgrund: Vaccinationer är en del av skolsköterskors förebyggande arbete. Stickrädsla är en vanlig åkomma, upp till 10 % av befolkningen i världen uppskattas ha det. Skolsköterskor möter någon gång under sin tid, stickrädda elever. God information innan vaccinationen har visat att stickrädsla minskar. Syftet: Att belysa skolsköterskors upplevelse av att vaccinera stickrädda elever. Metod: Studien genomfördes med en kvalitativ metod. Semistrukturerade intervjuer genomfördes med åtta skolsköterskor i två mindre städer, i mellersta Sverige. Resultatet har analyserats med innebördsanalys. Resultat: Analysen resulterade i fyra rubriker till resultatet; Skolsköterskors förhållningssätt påverkar bemötandet av elever, En inbjudande miljö påverkar hur vaccinationen upplevs av skolsköterskor och elever, Föräldrars förhållningssätt kan hjälpa eller stjälpa skolsköterskor vid vaccination, Avledning vid vaccination underlättar arbetet för skolsköterskor. Konklusion: Skolsköterskor måsta ha en god kunskap om hur olika faktorer kan påverka den stickrädda elever. Skolsköterskor upplever ofta en tidspress vilket påverkar mötet med stickrädda elever som i sin tur påverkar skolsköterskors vaccinationer. Skolsköterskor upplever att belöning har en god effekt för att lugna elever och underlätta för skolsköterskor att utföra sitt arbete. Hur föräldern är delaktig är betydelsefullt för skolsköterskors upplevelse. Det finns tre olika förhållningssätt bland föräldrar, den tysta, den bestämda och den tveksamma som låter elever själv välja. Dessa påverkar vaccinationstillfället och upplevelsen av mötet för verksamma skolsköterskor.

    Abstract [en]

    During roughly forty years of a woman’s life-span, the fertile female human body prepares itself monthly for the possibility of pregnancy. Science has shown that the fluctuation of the sex steroids progesterone and estrogen have a crucial role in the female body's physiology, determining the menstrual cycle and its general phases. This biological dance of hormones governing the cycle influences a lot of physical, mental and cognitive aspects of life for a fertile ovulating woman. Although the question of whether these changes also affect women's cognitive performance is still unclear, some evidence has been gathered that could bring us closer to answers. Recent research findings show that this hormonal interplay might have a significant role in cognitive and psychological development - modulating brain activity, cognitive performance, higher cognition, emotional status, sensory processing, appetite and more. This thesis aims to uncover to what extent the menstrual cycle affects brain functions, neurobiology, mood, well-being and cognitive performance in menstruating cisgender women.

    Abstract [en]

    Objective:  to explore how first-time fathers describe requested and received support during a normal birth. Design:  qualitative research design. Ten first-time fathers were interviewed during the first postpartum week.  Individual  open-ended  interviews  were  used  to  explore  the  fathers’  descriptions,  and  the interviews were analysed using qualitative analysis. Setting:  a labour ward at one hospital in a south-western county of Sweden in November and December 2006. Participants:  first-time  fathers  who  had  experienced  a  normal  birth  at  the  hospital  during  the study period. Findings:  the support described is presented as one main theme, ‘being involved or being left out’, which included four underlying categories: ‘an allowing atmosphere’, ‘balancing involvement’, ‘being seen’ and ‘feeling left out’. Key conclusions:  fathers perceived that they were given good support when they were allowed to ask questions during labour, when they had the opportunity to interact with the midwife and their partner, and when they could choose when to be involved or to step back. Fathers want to be seen as individuals who are part of the labouring couple. If fathers are left out, they tend to feel helpless; this can result in a feeling of panic and can put their supportive role of their partner at risk. Implications  for practice:  the  results  of  this  study could  initiate  discussions  about  how  health-care professionals can develop support given to the labouring couple, with an interest in increasing paternal involvement.

    Background: Professional support does not always meet the needs of expectant fathers or co-mothers. The way in which professional support is offered during pregnancy varies internationally, depending on the country. In order to attain a greater understanding of partners' experiences of professional support, it is necessary to further illuminate their perceptions of it. The aim of this study was therefore to explore pregnant women's partners' perceptions of professional support during pregnancy. Methods: Qualitative research design. Partners of pregnant women were interviewed during gestational week 36-38. Individual semi-structured interviews were used to explore the partners' perceptions. The data was analysed using a phenomenographic approach. The study was performed in a county in south-western Sweden; the data collection was conducted from November 2014 to February 2015. Fourteen partners (expectant fathers and co-mothers) of women who were expectant first-time mothers with singleton pregnancies, were interviewed. Results: The findings of the study are presented through four descriptive categories: Ability to absorb adequate information; Possibility to meet and share with other expectant parents; Confirmation of the partner's importance; and Influence on the couple relationship. Using a theoretical assumption of the relationship between the categories showed that the fourth category was influenced by the other three categories. Conclusions: The partners perceived that professional support during pregnancy could influence the couple relationship. The partners' ability to communicate and to experience togetherness with the women increased when the expectant couple received professional support together. The support created also possibilities to meet and share experiences with other expectant parents. In contrast, a lack of support was found to contribute to partners' feelings of unimportance. It was essential that the midwives included the partners by confirming that they were individuals who had different needs for various types of professional support. The partners perceived it easier to absorb information when it was adequate and given with a pedagogic that made the partners become interested and emotionally engaged.

    Abstract

    The surface of multicellular organisms is covered with epithelial cells that provide a barrier to the external environment. As part of this barrier function, most epithelia produce apical extracellular matrices (aECMs). The generation of such chemical and physical barriers requires specialized deposition of macromolecules and is likely to involve a spatial and temporal coordination of biochemical activities at the apical surface. A challenging task is thus to characterize key proteins that underlie apical cell surface organization and correct aECM assembly. The Drosophila trachea provides an excellent system to study aECM formation, as they produce an ordered aECM, called the cuticle. The tracheal cuticle is unique by its presence of cuticular ridges, called taenidial folds, which prevent collapse of tracheal tubes while allowing them to expand and contract along their length. A gene called filzig encodes a transmembrane serine protease and is required for taenidial organization. The aim of this research was to further understand Filzig function through characterization of filzig mutants and Filzig protein expression. The results showed that Filzig is expressed in cuticle-producing epithelia as cuticle deposition begins. Moreover, Flz localized to the apical epithelial surface, as well as to the aECM. The apical Flz localization does not reflect the pattern of cuticle ridges, indicating that Flz-localization is not a determinant for taneidial patterning. Instead, Flz might act on extracellular targets that localize to the future taneidial folds. Alternatively, Filzig is involved in a cascade of self-organizing activity of cuticular components to form the regular taenidial folds.

    Bakgrund: Cirka 20 % av Sveriges kvinnor uppger att de någon gång i livet blivit utsatta för våld i nära relationer. Misshandel kan uttrycka sig både fysiskt och psykiskt, vilket skapar ett lidande hos kvinnan vars självbild blir förvriden. Hälso- och sjukvården kan komma i kontakt med dessa kvinnor och kan ha en betydande roll för deras upplevelser av stöd och hjälp. Syfte: Syftet var att beskriva kvinnors erfarenheter av att leva i en våldsutsatt relation. Metod: En induktiv ansats valdes där datamaterialet bestående av självbiografier analyseras med kvalitativ innehållsanalys. Resultat: Ur analysen av datamaterialet framkom tre kategorier så som; börjar skönja partners våldsamma egenskaper trots ett kärleksfullt förhållande, känslomässigt kaos och vägen ur våldet och den efterföljande processen med nio underkategorier. Slutsats: Genom kvinnors erfarenheter av att leva i våldsutsatta relationer kan sjuksköterskan få fördjupad kunskap kring ämnet som kan bidra till att skapa tillit och tro för dessa kvinnor i mötet med vården.

    To address the research gap on age-based differences in health information literacy (HIL), we investigated how younger (born 1960–) and older adults (1946–1960), and elderly citizens (–1945) differed from each other by their HIL competencies. Data were collected with an online survey of patients using the Swedish national electronic health record system. Altogether, 2,587 users responded. One-way ANOVA with post hoc tests revealed several differences between the groups: younger adults were less likely to value health information than older adults; older adults and elderly were least likely to compare information from multiple sources and had trouble in determining health information needs; older adults were most likely to have trouble understanding health terminology and the elderly to have difficulties in understanding medicinal package labels. The study shows that HIL is not necessarily improving or declining but adapting to challenges of advanced age. © 2019, Springer Nature Switzerland AG.

    Bakgrund: Varje år ställs sjuksköterskor inför diverse problemställningar gällande palliativ vård i livets slutskede. Nutritionsbehandling är en av de åtgärder som kan sättas in för att lindra situationen för både patient och anhöriga. Då det är flera parter så som patient, anhöriga, läkare och sjuksköterska inblandade är det av stort vikt att alla förstår och är informerade om situationen för att den svårt sjuke ska få bästa möjliga vård. Syfte: Syftet med litteraturstudien är att beskriva sjuksköterskors upplevelser av nutritionsproblem hos patienter i livets slutskede. Metod: En litteraturstudie genomfördes där 10 kvalitativa och kvantitativa artiklar granskades och analyserades enligt Friberg (2012). Resultat: Det framkom tre olika kategorier som är avgörande för hur vården blir: Etiskt övervägande, där hänsyn tas till patient och anhöriga. Kommunikation, där sjuksköterskan spelar en avgörande roll som sammanlänkad mellan patient/anhöriga och läkare. Handledning och samarbete, här är individuella vårdplaner en viktig framgångsfaktor liksom ett nära samarbete mellan alla inblandande. Konklusion: Avgörande för en bra palliativ vård där nutritionsbehandling ingår är sjuksköterskans roll i att förmedla information från patient/anhöriga till läkare för att ge bästa förutsättningar för att kunna sätta in rätt vård, eller för att avstå viss behandling.

    Learning to live with diabetes in such a way that the new conditions will be a normal and natural part of life imposes requirements on the person living with diabetes. Previous studies have shown that there is no clear picture of what and how the learning that would allow persons to incorporate the illness into their everyday life will be supported. The aim of this study is to describe the phenomenon of support for learning to live with diabetes to promote health and well-being, from the patient’s perspective. Data were collected by interviews with patients living with type 1 or type 2 diabetes. The interviews were analysed using a reflective lifeworld approach. The results show that reflection plays a central role for patients with diabetes in achieving a new understanding of the health process, and awareness of their own responsibility was found to be the key factor for such a reflection. The constituents are responsibility creating curiosity and willpower, openness enabling support, technology verifying bodily feelings, a permissive climate providing for participation and exchanging experiences with others. The study concludes that the challenge for caregivers is to create interactions in an open learning climate that initiates and supports reflection to promote health and well-being.

    Abstract [en]

    Background: Surgery and diseases modify inflammatory responses and the immune system. Anesthetic agents also have effects on the human immune system but the responses they induce may be altered or masked by the surgical procedures or underlying illnesses. The aim of this study was to assess how single-drug dexmedetomidine and propofol anesthesia without any surgical intervention alter acute immunological biomarkers in healthy subjects. Methods: Thirty-five healthy, young male subjects were anesthetized using increasing concentrations of dexmedetomidine (n = 18) or propofol (n = 17) until loss of responsiveness (LOR) was detected. The treatment allocation was randomized. Multi-parametric immunoassays for the detection of 48 cytokines, chemokines and growth factors were used. Concentrations were determined at baseline and at the highest drug concentration for each subject. Results: The changes in the concentration of eotaxin (decrease after dexmedetomidine) and platelet-derived growth factor (PDGF, increase after propofol) were statistically significantly different between the groups. Significant changes were detected within both groups; the concentrations of monocyte chemotactic protein 1, chemokine ligand 27 and macrophage migration inhibitory factor were lower in both groups after the drug administration. Dexmedetomidine decreased the concentration of eotaxin, interleukin-18, interleukin-2Ra, stem cell factor, stem cell growth factor and vascular endothelial growth factor, and propofol decreased significantly the levels of hepatocyte growth factor, IFN-.-induced protein 10 and monokine induced by IFN-gamma, and increased the levels of interleukin-17, interleukin-5, interleukin-7 and PDGF. Conclusions: Dexmedetomidine seemed to have an immunosuppressive effect on the immune system whereas propofol seemed to induce mixed pro- and anti-inflammatory effects on the immune system. The choice of anesthetic agent could be relevant when treating patients with compromised immunological defense mechanisms. Trial registration: Before subject enrollment, the study was registered in the European Clinical Trials database (EudraCT number 2013-001496-21, The Neural Mechanisms of Anesthesia and Human Consciousness) and in ClinicalTrials.gov (Principal Investigator: Harry Scheinin, number NCT01889004, The Neural Mechanisms of Anesthesia and Human Consciousness, Part 2, on the 23rd of June 2013).

    Bakgrund: Barn med neuropsykiatriska funktionsnedsättningar (NPF) är i behov av stöd och det centrala är att känna sig sedd i hälso- och sjukvården. Skolsköterskans primära uppgift är att stödja och stärka till välbefinnande genom att vara närvarande, lyssna och inge hopp i samtalet.

    Syfte: Att beskriva skolsköterskans upplevelse av att stödja och stärka elever i skolan med neuropsykiatriska funktionsnedsättningar Metod: I studien användes en kvalitativ metod med induktiv ansats där åtta intervjuer analyserades med kvalitativ innehållsanalys.

    Resultat: Ur analysen framkom två huvudkategorier; Att inge trygghet och hopp genom tillgänglighet och samtal och Genom samverkan med andra utvecklas ett starkare stöd, med fem tillhörande underkategorier.

    Slutsats: Skolsköterskans stöd leder till att eleverna med NPF känner sig stärkta i sin situation vilket ger ökat hopp och tro på framtiden. Att hjälpa eleverna att främja dess sunda och friska beteenden medverkar till att elevernas skyddsfaktorer stärks. Genom samtal och lyhördhet stärks eleverna att lära sig ta egna beslut och växa som individer. Ett bra teamsamarbete bidrar till större möjlighet att främja hälsa och förebygga ohälsa för elever med NPF

    IntroductionThe highly selective α2-agonist dexmedetomidine has become a popular sedative for neurointensive care patients. However, earlier studies have raised concern that dexmedetomidine might reduce cerebral blood flow without a concomitant decrease in metabolism. Here, we compared the effects of dexmedetomidine on the regional cerebral metabolic rate of glucose (CMRglu) with three commonly used anaesthetic drugs at equi-sedative doses.

    MethodsOne hundred and sixty healthy male subjects were randomised to EC50 for verbal command of dexmedetomidine (1.5 ng ml−1n=40), propofol (1.7 μg ml−1n=40), sevoflurane (0.9% end-tidal; n=40) or S-ketamine (0.75 μg ml−1n=20) or placebo (n=20). Anaesthetics were administered using target-controlled infusion or vapouriser with end-tidal monitoring. 18F-labelled fluorodeoxyglucose was administered 20 min after commencement of anaesthetic administration, and high-resolution positron emission tomography with arterial blood activity samples was used to quantify absolute CMRglu for whole brain and 15 brain regions.

    ResultsAt the time of [F18]fluorodeoxyglucose injection, 55% of dexmedetomidine, 45% of propofol, 85% of sevoflurane, 45% of S-ketamine, and 0% of placebo subjects were unresponsive. Whole brain CMRglu was 63%, 71%, 71%, and 96% of placebo in the dexmedetomidine, propofol, sevoflurane, and S-ketamine groups, respectively (P<0.001 between the groups). The lowest CMRglu was observed in nearly all brain regions with dexmedetomidine (P<0.05 compared with all other groups). With S-ketamine, CMRgludid not differ from placebo.

    ConclusionsAt equi-sedative doses in humans, potency in reducing CMRglu was dexmedetomidine>propofol>ketamine=placebo. These findings alleviate concerns for dexmedetomidine-induced vasoconstriction and cerebral ischaemia.

    Bakgrund: Stressrelaterad ohälsa är idag vanligt förekommande och orsakar personligt lidande. Stress handlar om obalans mellan belastning och återhämtning. Långvarig stress kan resultera i en livssituation som ger både psykisk och fysisk ohälsa. För den som arbetar inom människovårdande yrken är det därför viktigt att kunna känna igen stress och dess symtom Syfte: Att belysa hur människor med stress upplever vistelse i naturen. Metod: Vald metod är en litteraturöversikt där kvalitativa och kvantitativa artiklar studerats. Resultat: Naturvistelse bidrar till psykologisk återhämtning vid stress. Stressade människor upplever att de har lättare att få ihop vardagen tack vare den återhämtning de får i naturen. Förmågan att reflektera kring sitt livssammanhang ökar och kraft och ambition att ta sig an framtiden infinner sig genom naturvistelse. Även fysisk återhämtning upplevs genom vistelse i naturen. Att erfara naturen via syn, känsel, smak och lukt upplevs ha en lugnande och avslappnande inverkan. Generellt bidrar vistelse i naturen till att människor känner sig förankrade i tillvaron och upplever känslor som glädje och tacksamhet. Konklusion: Resultatet visar att människor som lider av stress upplever återhämtning genom vistelse i natur

    Inledning: Stressrelaterad ohälsa är ett växande folkhälsoproblem som skapar lidande för individer och stora kostnader för samhället. Det finns behov av hälsofrämjande och förebyggande metoder för att motverka denna ohälsa. MBSR är en metod som visat lovande effekter på stressreducering och olika dimensioner av stressrelaterad ohälsa.                    

    Syfte: Syftet med litteraturöversikten var att beskriva effekter av Mindfulness Based Stress Reduction (MBSR) på ohälsosam stress i friska populationer.                                                                                                

    Metod: En litteraturöversikt med tio vetenskapliga artiklar som analyserades systematiskt för att identifiera teman i effekter av MBSR-program på stressrelaterad ohälsa.                                                                                                                   

    Resultat: Samtliga vetenskapliga studier visar hur MBSR-programmet ger positiva effekter på hälsan som ökad mindfulness, reducerad stress samt bättre psykisk och emotionell hälsa.

    Diskussion: MBSR har i tidigare studier visat sig vara effektiv för ökad livskvalitet och copingförmåga vid kroniska smärttillstånd samt olika fysiska och psykiska diagnoser. Mindre uppmärksamhet har riktats mot möjliga fördelar hos friska populationer. Resultatet av litteraturöversikten visar hur MBSR är en lovande metod för att främja hälsa och förebygga stressrelaterad ohälsa hos friska populationer i det korta perspektivet, vilket stöds av tidigare publicerade studier.

    The purpose of this article is to analyse the phenomenon of supportive care for older persons' well-being. The phenomenon is seen from the eldercarers' meaning-making through their lifeworld perspective at a residential care home. Based on primary empirical interview material with twelve professionals in the context of Swedish eldercare, a phenomenological analysis was undertaken. The result shows that the phenomenon of supportive care for older persons' well-being creates certain ambiguities in the professionals' meaning-making. In practice, it balances between the older persons' (from hereon called residents) needs and the conditions of the eldercare organization. The ambiguities (the what) is made up by three constituents: (i) freedom of choice for the older persons vs. institutional constraints, (ii) the residents' need for activation vs. wanting not to be activated, and (iii) the residents' need for routine vs. the eldercarers' not being able to know what the residents need. The conclusions drawn are that this ambiguity has consequences for the eldercarers' choice of handling supportive care for older persons' well-being (the how). They have to navigate between the support for authenticity, dwelling and mobility, and their own presence and time. In performing supportive care for older persons' well-being, the eldercarers have to consider aspects concerning the resident's lifeworld, the social setting of the eldercare ward, and the institutional demands of the organization. The practical implications for supporting well-being in the care of older residents are manifested in the importance of 'the little things', and the eldercarer's ability to give receptive attention, which requires presence.

    BACKGROUND: Negative views towards ageing and older adults may be a reason why nurses do not choose to work in gerontological nursing. Studying in another cultural context can challenge these views. The Objective was to explore nursing students' views on ageing and older adults before and after a gerontology course held abroad.

    DESIGN AND METHOD: A qualitative approach based on content analysis of responses to open-ended questions by 30 Thai nursing students studying a gerontology course in Sweden.

    RESULTS: Three main categories: positive imprints of ageing, ageing takes its toll, and knowledge leading to action, emerged through sub-categories carrying a view of older adults as not only in need of care, but also as resourceful and competent. Professional healthcare, besides family was seen as potential caregivers in old age.

    CONCLUSIONS: Studying gerontology abroad can widen views towards ageing and older adults, inspiring nurses to work in gerontological nursing.

    Bakgrund: Övervikt och fetma är en av de viktigaste enskilda bidragande dödsorsakerna och stod 2018 för 2,6 miljoner dödsfall i världen. I Sverige beräknas 18 - 20% av tioåringarna vara överviktiga och hos dessa har 3- 4% har fetma.

    Syfte: Syftet med denna studie är att visa på faktorer och insatser som kan främja fysisk aktivitet och förebygga övervikt och fetma bland barn och ungdomar.

    Metod: Arbetet är en litteraturstudie baserad på en artikelsökning i databaserna PubMed, och begränsad till publikationsåren 2000-2019. Tio stycken artiklar användes och målgruppen var ungdomar mellan 1 – 18 år.

    Resultat: Det huvudsakliga resultatet presenterades i två temaområden: 1. Faktorer som påverkar barn och ungdomar till fysisk aktivitet. 2. Insatser för att främja fysisk aktivitet och förebygga övervikt och fetma bland barn och ungdomar. Studier visade att en kombination av fysiska aktiviteter och andra insatser såsom hälsosamma måltidsvanor samt förmedling av övervikt och fetma till föräldrar och lärarna ger större effekter än andra typer av insatser som fokuserar enbart på fysiska aktiviteter eller bara måltidsvanor för att förebygga övervikt-fetma bland barn och ungdomar.

    Slutsats: Trots att övervikt och fetma är en komplex folkhälsofråga har det visat sig att fysiska aktiviteter som kombineras med andra insatser som inkluderar måltidsvanor, förmedla kunskap om övervikt och fetma till föräldrar och lärarna har förebyggande effekter mot övervikt bland barn. Men fler studier behövs för att få en förståelse av hur den fysiska aktiviteten bör utformas för att vara mest effektiv för att förebygga övervikt, samt för att klarlägga faktorer som främjar fysisk aktivitet hos barn och ungdomar.

    Conceptualizing and contextualizing mindfulness: New and critical perspectives2015Doktorsavhandling, sammanläggning (Övrigt vetenskapligt)

    This dissertation aims at analyzing mindfulness as a concept and a multidimensional phenomenon in its historic and primordial but also contemporary contexts. In the course of examining this more general question, this dissertation targets four specific objectives: 1) classifying existing definitions of mindfulness, 2) critically analyzing and interpreting the Buddhist and Western interpretations and practices of mindfulness, 3) elaborating on the social and existential dimensions of mindfulness, and 4) applying these dimensions in advancing the notion of mindful sustainable aging in the context of successful aging. Paper I examines and assesses the numerous definitions of mindfulness that have been presented over the years by a wide range of scholars from a variety of disciplines. Paper II traces the roots of modern mindfulness in Buddhism. It continues by exploring the utility and practices of mindfulness in the context of social work. The definitions provided in Paper I and the Buddhist underpinnings discussed in Paper II call attention to the fact that in addition to the more commonly considered physical and mental dimensions, mindfulness contains a social and an existential dimension as well – dimensions that remain under-researched and not well understood. To redress this imbalance, Paper III elaborates on these two latter dimensions, emphasizing their potential to enhance health, wellbeing and meaning in life. Paper III further argues that a more nuanced understanding of physical, mental, social and existential mindfulness can be obtained by examining the interconnectedness of all four fields. Paper IV continues the discussion of the social and the existential dimensions of mindfulness with specific emphasis on their utility for successful aging, and advances the notion of mindful sustainable aging. Paper IV highlights the potential of mindfulness for living a meaningful life and boosting the elderly’s capacity to find deeper meaning in their final stage of life.

    The primary aim of this article is to present a new concept called mindful sustainable aging (MSA), which is informed by mindfulness practices that support the physical, the mental, and especially, the social and the existential dimensions of old life. The concept of MSA is discussed and compared with four influential psychosocial theories in the field of gerontology, i.e., activity theory, disengagement theory, successful aging theory and gerotranscendence theory. The article ends with reviewing research on how mindfulness practice can help to manage, diminish and/or improve a number of serious physical conditions that are common among older people. The potential of mindfulness when it comes to facilitating for older adults in their quest for spiritual and existential meaning is discussed extensively throughout the article.

    Abstract [en]

    This thesis has examined what long-term psychological and neurological effects that are apparent in children and adults who have experienced parental divorce. It was predicted that significantly more children and adult children from divorced families would have increased symptoms of mental disorders than children and adult children from married homes e.g., anxiety, depression, panic disorder, and generalized anxiety disorder. It was further predicted that parental divorce would negatively affect the neurological system in the offspring. The correlation between children of divorce and negative neurological effects was not found to be true. However, adult children of divorce have significantly lower baseline cortisol levels compared to adult children of marriage. Dysregulated cortisol levels are highly associated with the development of e.g., anxiety, depression, and brain damage. Parental divorce did not only influence how secretion of the hormone cortisol is regulated within adult children of divorce, but how both children and adult children of divorce psychologically adapt postdivorce. Children of divorce have for instance lower general well-being, more symptoms of anxiety and depression, lower self-esteem, and feel more stress than children of marriage. Adult children of divorce are more likely to experience marital discord, getting divorced themselves, anxiety and depression, lower academic performance, and substance abuse, etc. The result of the thesis suggests that children and adult children from divorced families are negatively affected, both psychologically and neurologically, regardless of age. Parental divorce and supplementary effects make it more likely for children and adult children to experience more symptoms of mental disorders.

    Abstract [en]

    The Escherichia coli BarA and UvrY proteins were recently demonstrated to constitute a novel two-component system, although its function has remained largely elusive. Here we show that mutations in the sensor kinase gene, barA, or the response regulator gene, uvrY, in uropathogenic E. coli drastically affect survival in long-term competition cultures. Using media with gluconeogenic carbon sources, the mutants have a clear growth advantage when competing with the wild type, but using media with carbon sources feeding into the glycolysis leads to a clear growth advantage for the wild type. Results from competitions with mutants in the carbon storage regulation system, CsrA/B, known to be a master switch between glycolysis and gluconeogenesis, led us to propose that the BarA-UvrY two-component system controls the Csr system. Taking these results together, we propose the BarA-UvrY two-component system is crucial for efficient adaptation between different metabolic pathways, an essential function for adaptation to a new environment.

    Abstract [en]

    BarA is a membrane-associated protein that belongs to a subclass of tripartite sensors of the two-component signal transduction system family. In this study, we report that UvrY is the cognate response regulator for BarA of Escherichia coli. This conclusion is based upon homologies with analogous two-component systems and demonstrated by both biochemical and genetic means. We show that the purified BarA protein is able to autophosphorylate when incubated with [gamma-(32)P]ATP but not with [alpha-(32)P]ATP or [gamma-(32)P]GTP. Phosphorylated BarA, in turn, acts as an efficient phosphoryl group donor to UvrY but not to the non-cognate response regulators ArcA, PhoB, or CpxR. The specificity of the transphosphorylation reaction is further supported by the fact that UvrY can receive the phosphoryl group from BarA-P but not from the non-cognate tripartite sensor ArcB-P or ATP. In addition, genetic evidence that BarA and UvrY mediate the same signal transduction pathway is provided by the finding that both uvrY and barA mutant strains exhibit the same hydrogen peroxide hypersensitive phenotype. These results provide the first biochemical evidence as well as genetic support for a link between BarA and UvrY, suggesting that the two proteins constitute a new two-component system for gene regulation in Escherichia coli.

    Abstract [en]

    What it is like when a lifelong twin relationship ends through death in later life is the focus of this study. It draws on interview data from seven twins who are part of a longitudinal Swedish twin study (SATSA) and who lost their co-twins in old age. Data were analyzed using qualitative latent content analysis. The results showed that the experience of loss of the co-twin was pro-found, including an emotional as well as a be-havioral dimension. Loss and loneliness were expressed as the dominant feelings related to the quality of the missing relationship as well as the loss of twin identity. However, the grief ex-periences in this study were primarily related to the closeness and quality of the twin relation-ship, rather than identity. Behavioral adjust-ments included the use of outside as well as internal cognitive resources to cope with life after the loss. Despite the devastating experi-ence of losing a co-twin after a lifelong rela-tionship, the participants engaged actively in their own grief processes. It was concluded that twin loss is unique, in the sense of losing the relational twin identity, as well as it is charac-terized by similar features as the loss of a close relationship among non-twins.

    Background

    Worldwide, there is a growing population of older people who develop dementia in a country other than that of their origin. When their dementia has reached an advanced stage, residential care is most often needed. People with dementia in Sweden are often cared for in group homes. For immigrants, this may mean a linguistically challenging care environment for both healthcare staff and the patients’ family members.

    The aim of this study was to explore and describe the experiences of family members and professional caregivers regarding the care provided to immigrants with dementia in group homes in Sweden.

    Methods

    An exploratory, descriptive study with a qualitative approach was chosen. In-depth semi-structured interviews were conducted with nine professional caregivers and five family members of people with dementia with Finnish, Estonian, Hungarian and Ingrian backgrounds; all were chosen purposefully. All people with dementia had lost their Swedish language skills as their second language. The data was analysed using qualitative content analysis.

    Results

    Three main categories and seven subcategories were identified. The first main category: A new living situation comprised the subcategories: adjusting to new living arrangements and expectations regarding activities and traditional food at the group home, the second main category: Challenges in communication with the subcategories: limited communication between the immigrant with dementia and the Swedish-speaking nursing staff and the consequences of linguistic misunderstandings and nuanced communication in a common language and the third main category: The role of the family member at the group home with the subcategories: a link to the healthy life story of the family member with dementia andan expert and interpreter for the nursing staff.

    Conclusions

    The family member played a crucial role in the lives of immigrants with dementia living in a group home by facilitating communication between the nursing staff and the PWD and also by making it possible for PWD to access the cultural activities they wanted and which professional caregivers were either not able to recognise as needed or could not deliver.

    Det övergripande syftet med avhandlingen var att utforska, beskriva och förstå upplevelser av tvillingskap utifrån äldre tvillingars livsberättelser. De 35 tvillingarna som ingår i denna avhandling var ett urval av deltagarna i två longitudinella studier av äldre tvillingar, SATSA (Swedish Adoption Twin Study of Aging) och Gender. Studiedesignen är kvalitativ och vid intervjuerna användes narrativ metod och berättelsematerialet analyserades med narrativ analys (studie I och II) och med kvalitativ, latent innehållsanalys (studie III och IV).

    Enligt tvillingarna i denna avhandling beskrevs tvillingskapet utifrån relationen med tvillingpartnern (I, III, IV) och ur ett identitetsperspektiv (II). Tvillingrelationer är unika men representerar också variationer. Tre slags relationsmönster identifierades, benämnda den närande, tärande och den ytliga relationen som baseras på kvalitativa aspekter (I). De olika typerna av relationer blev särskilt tydliga under viktiga skeden i livet, såsom vid ingående av äktenskap (III) och vid förlusten av tvillingpartnern (IV), vilket innebar att tvillingarna fick anpassa sig till ett mer individualiserat liv. I den närande och den ytliga tvillingrelationen upplevdes inte dessa livsförändringar som särskilt dramatiska, medan de för tvillingarna i den tärande relationen upplevdes som mer dramatiska. Från ett anknytningsteoriskt perspektiv förblev de äldre tvillingarna ankytningspersoner med oförändrade anknytningsmönster genom livet (I). Sammanbundet med den nära tvillingrelationen är hur tvillingar definerar sig själva, då tvillingskapet innebär att hantera både sin individuella identitet och tvillingidentiteten. Självbeskrivningarna med betoning av olikheter, ses mot bakgrund av hur tvillingarna upplevde att omgivningen uppfattade dem som en social enhet och tolkades som en vilja att betona sin individualitet i förhållande till tvillingpartnern och som ett budskap till omgivningen att vilja bli sedd som en egen individ (II).

    Sammanfattningsvis beskrevs tvillingskap av de flesta som en nära, berikande relation genom livet och för vissa mindre berikande beroende på vilken slags relation de hade till sin tvillingpartner. Parallellt pågick ett identitetsarbete att ta plats som en egen individ i tvillingrelationen och att hävda sin individualitet gentemot den övriga omgivningen i budskapet: ”Vi är inte så lika som ni tror”!

    Based on 35 life stories of aging twins, this study focuses on personal experiences and recollections of their relationships with the co-twin over the life course. The participants are part of two longitudinal Swedish twin studies on aging, SATSA and Gender. In the narrative analysis, three relationship patterns, labeled ‘nurturing’, ‘draining’, and ‘superficial’, emerged, pointing to qualitative aspects in the co-twin relationship. The dominating aspect was emotional closeness, which differed in the three relationship patterns. In the nurturing twin relationship pattern, emotional closeness was experienced as intimacy and yet independence, while in the draining relationship pattern it was experienced as dependence. The superficial twin relationship was experienced as distant and lacking in emotional involvement. Most of the relationship patterns seemed to remain the same throughout life. However, seen from a life course perspective, this study pointed to complexity and diversity in lifelong twin relationships.

    Abstract [en]

    Human parainfluenza virus type-2 (HPIV-2) is a highly contagious respiratory pathogen that can cause severe respiratory disease known as laryngotracheobronchitis or croup-like disease in children. No specific vaccine or an antiviral drug is currently approved for treatment of HPIV-2 infections. In this project, a library of 14400 diverse compounds had been screened for anti-HPIV-2 activities in cultures of African green monkey kidney cells. All compounds that inhibited the virus induced syncytium-forming activity in these cells were considered as hit compounds. Three hit compounds showed moderate anti-HPIV-2 activity characterized by the IC50 values of 20 µM and selectivity indices of approximately 5. This suggests that the antiviral activity of these compounds was due to targeting activities of cellular rather than viral components. Another hit compound, referred to as compound 5, showed anti-HPIV-2 activity that was manifested as a reduction of area of the virus-induced plaques in cells at not cytotoxic concentrations. Interestingly, this compound did not inhibit initial infection nor the virus production in infected cells as revealed by the time-of-addition assay. Moreover, it showed no direct the virus-inactivating (virucidal activity) against HPIV-2 particles. However, relatively short pre-treatment (4 hours) of the cells with compound 5 prior to the virus infection was sufficient for its plaque size-reducing activity suggesting that anti-HPIV-2 activity of compound 5 was due to targeting activities of cellular rather than viral components. Further studies are needed to elucidate the anti-HPIV-2 mechanism of activity of hit compounds identified in the present study.

    Bakgrund: Personer med demens har kognitiva nedsättningar. De får svårt att sköta sin egen munhygien, detta leder till en förvärring av den kroniska sjukdomen. Bemötandet är avgörande då de behöver känna tillit personen som hjälper till med munvården. Hinder uppstår vid kunskapsbrist och motstånd. Senior alert är hjälpmedel där dålig munhälsa kan identifieras. Syfte: Att beskriva vårdpersonalens erfarenheter av att främja munhälsan hos personer med demens. Metod: Litteraturöversikt med kvantitativ och kvalitativ ansats. Resultat: Två teman uppkom: Kunskap ger kompetens som innebär att kunskap om demens symtom, munhygien samt munvård är viktiga delar till en god munvård. En god kommunikation med gott samarbete personal emellan samt gentemot patienter är även detta viktiga delar. Hinder ger känsla av obehag innefattar när munvården blir svår att utföra då personal upplever organisatoriska hinder som tidsbrist och otillräcklig personal tillsättning samt hinder från patienter som verbalt och fysiskt motstånd. Konklusion: Kunskap om demens och munvård är viktigt för att kunna bibehålla en bra munhälsa där utbildning är nyckeln till kunskapen. Denna bör ges under utbildningen men även under arbetstiden. Även ges kontinuerligt då det sker personalomväxlingar och forskning går framåt.

    Abstract [en]

    Sepsis is the primary cause of death from infection. Globally, an estimated 18 million people die from sepsis annually, exceeding deaths caused by HIV/AIDS, breast cancer, and prostate cancer combined1. Early sepsis diagnosis and targeted antimicrobial (AM) therapy reduce the length of intensive care for patients and cost by 30%2. The current gold standard, using blood cultures (BC), takes 12-72 h to detect pathogens in the blood and even longer to identify the exact pathogen and its AM susceptibility for optimal therapy. The aim of this study was to determine whether the QuickFISHTMBC test (AdvanDx, Woburn, MA) could be robust, timesaving and specific in the clinical microbiology laboratory setting, for identification of the pathogens causing life-threatening bloodstream infections.

    We have explored how older twins experience and describe themselves in relation to their co-twin. The life stories of 20 older twins were analyzed with narrative analysis.

    Results showed that the twins described themselves from the point of differences in relation to the co-twin. This was based on experiences of how other people viewed them as alike, as well as on life events along the life course, which contributed to the perception of oneself as an individual in relation to the co-twin. The emphasis on unlikeness was therefore interpreted as a way of trying to establish a position as an individual within the co-twin relationship and to assert ones individuality to the rest of the social environment. To claim oneself as an individual was an ongoing identity work along the life course.

    In this article, we are concerned with narratives of elderly women’s well-being from their perspectives of the latter parts of their life, living at special housing accommodation (SHA) in the context of Swedish elderly care. In focusing on narratives about well-being, we have a two-fold focus: (1) how the elderly women create their own identity and meaning-making based on lifetime experience; and (2) how narratives of well-being are reflected through the filter of life in situ at the SHA. Based on empirical data consisting of well-being narratives, a dialogical performance analysis was undertaken. The results show how relationships with important persons during various stages of life, and being together and enjoying fellowship with other people as well as enjoying freedom and self-determination, are central aspects of well-being. The conclusions drawn are that the characteristic phenomena of well-being (the what) in the narratives are continuity, identity, and sociality for the elderly person, and this is manifested (the how) as a question of contrasting the state of self-management and self-decline.

    The total number of persons living with dementia is estimated to double every 20 years and ageing migrant populations are growing in several countries. There are gaps in the health and social care of people from other countries, regardless of the efforts made when someone has a dementia diagnosis; similarly, receiving care in sheltered accommodation is less common. The aim of this study was to explore and describe the nursing staff's experiences of caring for non-Swedish speaking persons living with dementia in a Finnish speaking group home in relation to a Swedish speaking group home in Sweden. 27 qualitative semi-structured interviews were analysed using qualitative content analyses. The first main category, "communication", concentrated on language abilities and deficiencies, non-verbal language, highlighting the consequences of not understanding and the benefits of a common language. The second main category, "culturally oriented activities", focused on being served traditional food, celebrating holidays at the group home, the importance of traditions and the importance of familiar music as cultural elements. The Swedish speaking nursing staff could provide qualitative and equitable care, but the challenge was greater for them than for the bilingual nursing staff who spoke the same language as the residents.

    Abstract [en]

    Abstract. Discrepancies in perceived parenting and parental roles across European countries could be due to the use of different assessment techniques or due to mean level differences in the authoritative, authoritarian, or permissive parenting styles. The present study aimed to examine the psychometric characteristics of the Parenting Styles and Dimensions Questionnaire (PSDQ; Robinson, Mandleco, Olsen, & Hart, 1995, 2001) in a sample of 225 Greek, 301 Italian, and 279 Swedish adolescents aged 16?19 years, who evaluated their father?s and mother?s parenting styles during their childhood. A confirmatory factor analysis (CFA), multigroup CFA, and modified version of the correlated uniqueness model were used to evaluate the structure and invariance of the scale across countries. Measurement and structural invariance was found in the 8-item authoritative scale and 6-item authoritarian scale. A mixed ANOVA (Country ? Style ? Role) showed that Swedish mothers scored lower than Italian or Greek mothers, and that, in the three countries, mothers were perceived as more authoritative than were fathers.

    Abstract [en]

    Tumor suppressor genes play a role as a growth regulator and a gatekeeper of a cell. Their inactivation is often detected in malignant tumors. Identification of novel tumor suppressor gene candidates may help to further understand tumorigenesis and aid in the discovery of a new treatment leading toward cure of cancer.

    This PhD research project aimed to understand functional significance of a novel tumor suppressor gene candidate, myosin IC (MYO1C) and to identify potential interaction(s) of the MYO1C protein with key components of the signaling pathways involving in cancer development.

    In an experimental rat model for endometrial carcinoma (EC), detailed molecular genetic analysis of a candidate tumor suppressor region located distal to the tumor protein 53 (Tp53) suggested the myosin IC gene (Myo1c) as the best potential target for deletion of the genetic material. The question arising was whether and how MYO1C could function as a tumor suppressor gene. By using qPCR, Western blot or immunohistochemistry analyses, we examined MYO1C protein level in panels of well-stratified human colorectal cancer (CRC) and EC respectively. We found that MYO1C was significantly down-regulated in these cancer materials and that for the EC panel, the observed down-regulation of MYO1C correlated with tumor stage, where tumors at more advanced stages had less expression of MYO1C. In cell transfection experiments, we found that over-expression of MYO1C significantly decreased cell proliferation, and silencing MYO1C with siRNA increased cell viability. Additionally, knockdown of MYO1C impaired the ability of cells to migrate, spread and adhere to the surface. Recent published studies suggested a potential interplay between MYO1C and the phosphoinositide 3-kinase (PI3K)/AKT pathway. To examine this hypothesis, we analyzed the expression and/or activation of components of the PI3K/AKT and RAS/ERK signaling pathways in vivo in CRC samples, and in vitro in cells transfected with the MYO1C gene expression construct or MYO1C-targeted siRNA. To identify other potential pathways/ mechanisms through which MYO1C may exert its tumor suppressor activity, we additionally performed new sets of MYO1C-siRNA knockdown experiments. At different time points post transfection, we performed microarray global gene expression experiments followed by bioinformatics analysis of the data. Altogether, the results suggested an early PI3K/AKT response to altered MYO1C expression. We additionally identified several cancer-related genes/pathways with late response to MYO1C knockdown. All things considered, the identification of MYO1C-expression impact on cell proliferation, migration, and adhesion in combination with its interplay between several cancer-related genes and signaling pathways provide further evidence for the initial hypothesis of a tumor suppressor activity of MYO1C.
