Abstract [sv]

Bakgrund: Brytpunktssamtal är svåra samtal för alla deltagande. Bemötandet och hur beskedet ges kan bli avgörande för patient och närståendes upplevelse av livets slut. Syfte: Syftet var att beskriva sjuksköterskors erfarenheter av brytpunktssamtal i kommunal hemsjukvård. Metod: Data analyserades med kvalitativ innehållsanalys med induktiv ansats. Sju intervjuer genomfördes med sjuksköterskor. Resultat: Ur analysen av datamaterialet framträdde fyra teman såsom; sträcker sig över tid och rum, teamet samlat runt patienten, eftersträvar att vara överens, höga krav på sjuksköterskan med 13 underteman. Konklusion: Ökad kunskap kring brytpunktssamtal, dess innehåll samt hur det dokumenteras behövs. Brytpunktssamtalet bör dessutom eventuellt omvärderas från att vara starkt kopplat till läkarnärvaro till att användas i hemsjukvården även när sjuksköterskorna utför liknande samtal själva. Kraven på sjuksköterskan ställer ytterligare anspråk på sjuksköterskeutbildningar att undervisa palliativ vård. Inom hemsjukvården bör arbetsledning erbjuda handledning, stöd och vidareutbildningar inom ämnet.

Abstract [en]

Background: End-of-life discussion is a difficult conversation for everyone involved. How information is given and patient and relatives are treated can determine their experiences of the end-of-life. Aim: The aim was to describe nurses’ experiences of end-of-life discussions in municipal home care. Method: The data were analyzed using qualitative content analysis technique with inductive approach. Seven interviews with registered nurses were performed. Results: Four themes emerged from the analysis; expanding over time and space, the team surrounding the patient, striving for everyone to get along and high demands on the nurse, with 13 subthemes. Conclusion: Further knowledge about end-of-life discussion, what it withholds and how it is to be documented, is needed. The definition of the concept might need to be reevaluated from being highly associated with doctor presence to also be used in municipal home care when nurses have similar conversations without the doctor. The high expectations and demands on nurses within this context requires for education programs to teach palliative care. The management of the municipal home care should offer guidance, support, and further training to their nurses.
