Perioperative hypothermia : An investigation of the incidence in day surgery patients (English)
Abstract [sv]

Bakgrund: Det finns många faktorer som försätter patienten i oavsiktlig hypotermi under operation. Redan under första timmen kan kroppstemperaturen sjunka med 0,3–1°C på grund av anestesi och förhållanden på operationssalen. Konsekvenserna blir försämrad sårläkning, större risk för sårinfektion, förlängd vårdtid samt obehag för patienten. Anestesi- och operationssjuksköterskan har som gemensam arbetsuppgift att förebygga och identifiera uppkomst av hypotermi.

Syfte: Att undersöka förekomst, samband och riskfaktorer för oavsiktlig hypotermi perioperativt.

Metod: En empirisk studie med kvantitativ, longitudinell, prospektiv ansats. Antal formulär som samlades in på dagkirurgisk avdelning var 224 och dessa analyserades deskriptivt och analytiskt.

Resultat: Resultatet visar att hypotermi förekom hos 30,4 % av patienterna och är vanligast förekommande vid överrapportering till postoperativa avdelningen. En fjärdedel av patienterna frös under vårdförloppet och de visar sig vara signifikant kallare än de som svarade nej på frågan. Temperatur sjunker signifikant vid stigande ålder. Den största riskfaktorn till att drabbas av hypotermi är låg temperatur vid ankomst.

Konklusion:Denna studie har givit ökad kunskap om förekomst, samband och riskfaktorer för hypotermi. En tredjedel av patienterna var hypoterma vilket visar att det finns förbättringsmöjligheter. Patienters temperatur vid ankomst visade sig ha stor betydelse för patientens fortsatta temperatur i det perioperativa förloppet. Medvetenhet av resultatet i studien kan bidra till förbättrad patientkomfort och minskad risk för komplikationer.

Abstract [en]

Background: There are many factors that put the patient in unintentional hypothermia during surgery. Already during the first hour, the body temperature can drop by 0.3–1°C due to anesthetic agents and conditions inside the operating room. The consequences are impaired wound healing, greater risk of wound infection, prolonged treatment time and discomfort for the patient. The anesthetic and surgical nurse have a common task to prevent and identify the onset of hypothermia.

Aim: To investigate the occurrence, correlations and risk factors for inadvertent hypothermia perioperatively.

Method: An empirical study with a quantitative, longitudinal, prospective approach. The number of forms that were collected on a day surgery department was 224 and these were analyzed descriptively and analytically.

Results: The results show that hypothermia occurred on 30.4% of the patients and is most common in time of handover to the postoperative department. A quarter of the patients felt cold during the time of care and they are found to be significantly colder than those who answered no to that question. Temperature decreases significantly with increasing age. The most important riskfactor for suffering from hypothermia is low temperature on arrival.

Conclusion: This study has provided increased knowledge about the occurrence, association and risk factors of hypothermia. A third of the patients where hypothermic, which shows that there is room for improvement. The patient's temperature on arrival was found to be of great importance for the patient's continued temperature in the perioperative course. Awareness of the results in the studies can help to improve patient comfort and reduce the risk of complications.
