Nothing compares to district nurses : The district nurse´s role within the project co-operative health care (English)
Abstract [sv]

Bakgrund: Antalet äldre människor ökar i Sveriges, de är ofta sköra, sjuka och har mer behov av vård och omsorg.  För att möta ett ökat vårdbehov startades ett samverkansprojekt, Samverkande sjukvård, för att förkorta vägarna inom sjukvården. Forskning visar att äldre blir sjukare av inläggning på sjukhus och efter besök på akutmottagning blir de ofta inlagda på sjukhus på grund av komplex sjukdomsbild. Syfte: Undersöka om distriktssköterskans profession inom projektet Samverkande sjukvård leder till ett minskat användande av andra sjukvårdsresurser. Metod: Studien är en retrospektiv studie med kvantitativ ansats. Redan insamlade enkätsvar tillämpades som datainsamlingsmetod. Analysen genomfördes med deskriptiv analys och sambandsanalys. Resultatet presenteras i tabeller, figurer och text. Resultat: Distriktssköterskans insatser leder i många fall till vård i hemmet och att det förebyggs besök till akutmottagning, vårdcentral/jourcentral och inläggning på sjukhus. Konklusion: Ett samarbete mellan olika sjukvårdsinstanser leder till förbättring för såväl patienter som personal.

Abstract [en]

Background: The number of elderly in Sweden is increasing. They are often fragile and sickly, needing more care and support. To meet an increased need of healthcare, a collaboration project, Collaborative Healthcare, was established to ensure that patients are treated at the right place. Research shows that the elderly often fall ill from hospital admission, and after visits to the emergency department, they are more often admitted because of their complex medical history. Aim: To investigate whether the district nurse's profession within the project Collaborative Healthcare leads to a reduction in the use of other healthcare resources. Method: This study is retrospective with a quantitative approach. Data collected by others within the project were used. The analysis was performed with descriptive and correlation analysis. The results are presented in tables, figures and text. Results: The district nurse's efforts often lead to nursing in the home, preventing visits to health centers and emergency rooms as well as hospital admissions. Conclusion: Cooperation between different healthcare authorities leads to benefits for both patients and staff.
